Non-alcoholic fatty liver disease: pathophysiological concepts and treatment options

Christoph Grander et al.

Summary: The prevalence of non-alcoholic fatty liver disease (NAFLD) is increasing due to global obesity epidemic. It is a systemic metabolic disease accompanied by insulin resistance and inflammation. While simple hepatic steatosis is the most common manifestation, around 10-20% of individuals with NAFLD have a more progressive disease course characterized by liver fibrosis and inflammation. NAFLD can progress to liver cirrhosis and hepatocellular carcinoma. Weight loss and lifestyle changes are the main treatments, and there are several promising medical treatment options currently under investigation.

Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD): A State-of-the-Art Review

Wah-Kheong Chan et al.

Summary: Metabolic dysfunction-associated steatotic liver disease (MASLD) is the latest term for steatotic liver disease associated with metabolic syndrome. It is the most common cause of chronic liver disease and liver-related morbidity and mortality. In tackling the public health threat of obesity and obesity-related diseases, including MASLD, it is important for all stakeholders to be involved. Simple and clear assessment and referral pathways using non-invasive tests are crucial to identifying and referring patients with severe MASLD to specialist care, while managing patients with milder disease in primary care.

Nonalcoholic Fatty Liver Disease in Patients with Type 2 Diabetes: Screening, Diagnosis, and Treatment

Stefano Ciardullo et al.

Summary: Nonalcoholic fatty liver disease (NAFLD), also known as metabolic dysfunction-associated steatotic liver disease (MASLD), is common in patients with type 2 diabetes (T2D) and can lead to advanced liver fibrosis. Patients with T2D have an increased risk of liver-related complications and mortality. Screening for NAFLD/MASLD and advanced liver fibrosis is recommended in T2D patients. While there are no approved drugs for NAFLD/MASLD, certain antidiabetic drugs have shown positive effects on liver health. Diabetologists have the opportunity to actively treat NAFLD/MASLD and potentially change its natural course.

Summary: In a systematic review and meta-analysis, it was found that women have a lower risk of NAFLD than men. However, once NAFLD is established, women have a higher risk of advanced fibrosis than men, especially after age 50 years.

Usefulness of neutrophil-to-lymphocyte ratio (NLR) as a prognostic predictor after treatment of hepatocellular carcinoma

Mohamad Mouchli et al.

Summary: The NLR has potential value in evaluating the prognosis of HCC patients in terms of survival, cancer recurrence, and graft survival, but there are differences in research results due to heterogeneity in study methods. Most studies indicate a correlation between high pre-treatment NLR levels and poor outcomes. NLR is a reliable and inexpensive biomarker that should be integrated into other prognostic models to guide outcomes following HCC treatment.

Role of Insulin Resistance in MAFLD

Yoshitaka Sakurai et al.

Summary: Metabolic dysfunction is closely related to the development of non-alcoholic fatty liver disease (NAFLD), prompting consideration of renaming it as metabolic dysfunction-associated fatty liver disease (MAFLD). Factors such as obesity, type 2 diabetes mellitus, hypertension, dyslipidemia, and metabolic syndrome contribute to metabolic dysfunction, leading to insulin resistance and fat accumulation in the liver. Genetic factors also play a role in hepatic fat accumulation and fibrosis, exacerbating inflammation, fibrosis, and carcinogenesis.

Inflammation and Fibrogenesis in MAFLD: Role of the Hepatic Immune System

Pietro Torre et al.

Summary: MAFLD is a newly proposed definition to better describe the spectrum of NAFLD, with progression driven by hepatic inflammation and fibrogenesis. Fibrosis is a dynamic process occurring in a microenvironment where it interacts with fibrogenic cells and elements including immune cells, yet there is currently no approved drug for treating this condition.

Non-alcoholic fatty liver disease: An overview of risk factors, pathophysiological mechanisms, diagnostic procedures, and therapeutic interventions

Samar H. Gerges et al.

Summary: NAFLD is a disorder characterized by excessive fat accumulation in the liver, which can progress to NASH, accompanied by inflammation and fibrosis. The pathophysiology of NAFLD and NASH involves a complex interplay between metabolic, environmental, and genetic factors. Currently, there is no FDA-approved treatment for NAFLD and NASH, with off-label medications and hepatoprotective agents being used instead.

Summary: NAFLD is a common cause of chronic liver disease in the Western world, often caused by factors such as obesity, metabolic diseases, and environmental agents. Progression from simple hepatic steatosis to NASH can lead to liver cirrhosis and complications. Understanding the mechanisms that cause this progression is crucial for controlling the advancement of the disease.

Metabolic-associated fatty liver disease and lipoprotein metabolism

Joerg Heeren et al.

Summary: MAFLD is characterized by pathological accumulation of triglycerides in hepatocytes and is closely related to disturbances in systemic energy metabolism. Liver plays a central role in lipid metabolism, with excessive hepatic triglyceride synthesis being a key factor in the development of MAFLD. Interventions targeting lipid storage and energy combustion in adipose tissues may help ameliorate MAFLD.

Summary: The study found that the reference interval for NLR, LMR, and PLR in Chinese healthy adults is influenced by gender and age, with the upper limits of NLR and PLR increasing with age in males but decreasing for LMR. In females, the reference upper limits showed fluctuations in different age groups, indicating a need for individualized assessment.

Summary: In this study, 720 COVID-19 patients were analyzed to determine the association of neutrophil to lymphocyte ratio (NLR) with disease severity and mortality. The results showed a significant relationship between NLR and disease severity as well as mortality, suggesting NLR as a potential marker for assessing COVID-19 severity and predicting outcomes.
