HuMSC-EV induce monocyte/macrophage mobilization to orchestrate neovascularization in wound healing process following radiation injury

Celine Loinard et al.

Summary: This study investigates the mechanisms by which human mesenchymal stem cell-derived extracellular vesicles (HuMSC-EV) promote angiogenesis and neovascularization after radiation injury. The study found that HuMSC-EV improved neovascularization of irradiated tissue by stimulating angiogenesis, increasing capillary density, and producing proangiogenic factors. HuMSC-EV also promoted vasculogenesis by recruiting and differentiating bone marrow progenitors. Furthermore, HuMSC-EV enhanced arteriogenesis by mobilizing monocytes and promoting their recruitment to the injured site. These findings suggest that HuMSC-EV could be a potential alternative to stem cells for therapeutic neovascularization in tissue repair.

MSC-sEV Treatment Polarizes Pro-Fibrotic M2 Macrophages without Exacerbating Liver Fibrosis in NASH

Bin Zhang et al.

Summary: Mesenchymal stem/stromal cell small extracellular vesicles (MSC-sEVs) have shown promise in treating various human diseases in animal models. However, the potential for contraindication in fibrotic diseases due to their ability to induce pro-fibrotic M2 macrophage polarization is an important consideration. Despite this, MSC-sEVs were found to be effective in reducing liver fibrosis in a mouse model of non-alcoholic steatohepatitis (NASH).

Mesenchymal Stromal Cell Exosomes Mediate M2-like Macrophage Polarization through CD73/Ecto-5′-Nucleotidase Activity

Kristeen Ye Wen Teo et al.

Summary: MSC exosomes promote the polarization of M2-like macrophages through activation of the toll-like receptor signaling pathway and CD73-mediated adenosine production. The production of adenosine activates AKT/ERK-dependent signaling pathways via binding to adenosine receptors A(2A) and A(2B). These findings highlight the critical role of CD73 in mediating M2-like macrophage polarization by MSC exosomes, and have implications for predicting the immunomodulatory potency of MSC exosome preparations.

Mesenchymal Stem/Stromal Cell-Based Therapies in Systemic Rheumatic Disease: From Challenges to New Approaches for Overcoming Restrictions

Bong-Woo Lee et al.

Summary: Systemic rheumatic diseases, such as rheumatoid arthritis, systemic lupus erythematosus, and systemic sclerosis, are chronic autoimmune diseases affecting multiple organs and tissues. Mesenchymal stem/stromal cell (MSC)-based therapy shows promise for treating these diseases due to the regenerative and immunomodulatory properties of MSCs. However, there are challenges related to MSC sourcing, characterization, standardization, safety, and efficacy that need to be addressed for effective clinical use.

Mesenchymal Stem Cell Exosomes Promote Functional Osteochondral Repair in a Clinically Relevant Porcine Model

Shipin Zhang et al.

Summary: This study validates the safety and efficacy of human mesenchymal stem cell (MSC) exosomes combined with hyaluronic acid (HA) for osteochondral repair in a clinically relevant micropig model. The results show that the combination treatment promotes functional cartilage and subchondral bone repair, with significantly improved morphological, histological, and biomechanical outcomes. No adverse response or major systemic alterations were observed, indicating the potential of this therapy for clinical applications.

Mechanism for the attenuation of neutrophil and complement hyperactivity by MSC exosomes

Jia Tong Loh et al.

Summary: This study identified a feed-forward loop between complements and neutrophils that contributes to the cytokine storm in severe SARS-CoV-2-infected patients. It was observed that the terminal complement activation complex C5b-9 directly triggers neutrophil extracellular trap (NET) release and interleukin (IL)-17 production by neutrophils. Mesenchymal stem cell (MSC) exosomes were found to inhibit this loop by suppressing complement activation through exosomal CD59, suggesting their potential role in alleviating immune dysregulation and inflammation in acute respiratory failure.

Summary: Mesenchymal stromal/stem cells (MSCs) have been approved for treating certain diseases through paracrine secretion of small extracellular vesicles (sEVs). Despite the potential advantages of sEV preparations over cellular MSC products, there are challenges in developing MSC-sEV drug products, including establishing quality control metrics and developing effective potency assays.

The Macrophage Response Is Driven by Mesenchymal Stem Cell-Mediated Metabolic Reprogramming

Noymar Luque-Campos et al.

Summary: MSCs, multipotent adult stromal cells, are widely studied for their regenerative and immunomodulatory properties, capable of modulating macrophage plasticity and function. Metabolic changes can also affect macrophage fate and function, prompting phenotype change in response to environmental cues. Understanding the crosstalk between macrophages and MSCs can improve therapeutic potential in inflammatory diseases, cancer, and tissue repair processes.
