The role of stereotactic radiotherapy in addition to immunotherapy in the management of melanoma brain metastases: results of a systematic review

Summary: This study aimed to evaluate the effectiveness and safety of stereotactic radiotherapy (SRT) in combination with immunotherapy for treating melanoma brain metastases (MBM) through a systematic literature review. The results showed that SRT plus immunotherapy had improved overall survival in melanoma cancer patients with brain metastases, but did not improve local control and disease-free survival rates. Only one study reported G3-G4 toxicity, which was similar between the SRT plus immunotherapy group and the immunotherapy group. However, the quality of evidence is very low. Therefore, the use of SRT plus immunotherapy should be evaluated on an individual basis after multidisciplinary team discussion.

The centrally restricted diffusion sign on MRI for assessment of radiation necrosis in metastases treated with stereotactic radiosurgery

Summary: The differentiation of radiation necrosis from tumor progression in brain metastases treated with stereotactic radiosurgery is challenging. The centrally restricted diffusion sign showed a high sensitivity and negative predictive value for diagnosing radiation necrosis in this study. In the absence of the centrally restricted diffusion sign, the probability of radiation necrosis was found to be low.
