A Clinical Update on Vasoactive Medication in the Management of Cardiogenic Shock

Aditi Shankar et al.

Summary: This focused review examines the use of pharmacological support in cardiogenic shock. While there is abundant data on vasopressors and inotropes for septic shock, the data for cardiogenic shock are limited. The review article provides a detailed description of the pathophysiology of cardiogenic shock, the mechanism of action of different vasopressors and inotropes, and highlights their indications and potential side effects. Additionally, it integrates currently used risk-prediction models and trial data on the use of vasopressors and inotropes in cardiogenic shock, and discusses the future direction for vasoactive medications in this condition.

Use of peripherally inserted central catheters with a dedicated vascular access specialists team versus centrally inserted central catheters in the management of septic shock patients in the ICU

Hassan A. Raza et al.

Summary: This study aimed to compare the feasibility, safety, and outcomes of using dedicated vascular access specialist (VAS) teams to insert peripherally inserted central catheters (PICCs) versus centrally inserted central catheters (CICCs) for patients with septic shock. The results showed that patients who received PICCs had shorter time to central line insertion and vasopressor initiation, as well as shorter ICU length of stay and lower unadjusted hospital mortality.

Mechanical Circulatory Support Devices for the Treatment of Cardiogenic Shock Complicating Acute Myocardial Infarction-A Review

Sharon Bruoha et al.

Summary: Cardiogenic shock complicating acute myocardial infarction is a complex clinical condition with poor prognosis. Early target vessel revascularization is the most effective treatment, but mortality remains high. Temporary circulatory support devices aim to enhance cardiac unloading and improve organ perfusion, but there is a lack of quality evidence to guide device selection and management, highlighting the importance of multidisciplinary expert care.

Utilization and extravasation of peripheral norepinephrine in the emergency department

Tammy T. Nguyen et al.

Summary: This study investigated the use of peripheral norepinephrine (pNE) in adult patients in the VCUHS ED and found that administration according to protocol resulted in a low extravasation rate. The majority of patients received pNE for distributive shock, and 84% of patients received a central line.

Cardiogenic Shock After Acute Myocardial Infarction A Review

Marc D. Samsky et al.

Summary: Cardiogenic shock affects 40,000 to 50,000 people in the US annually and is a leading cause of in-hospital mortality post acute myocardial infarction. Immediate revascularization of the infarct-related coronary artery is the primary treatment recommended, supported by clinical trials. Despite lack of definitive evidence, mechanical circulatory support devices are frequently utilized in managing cardiogenic shock.

Summary: CS remains the leading cause of mortality in patients with acute myocardial infarction, and early revascularization has shown survival benefit in treating CS patients. A recent classification scheme and scientific statement have recommended the establishment of CS centers with standardized protocols for diagnosis and management.

Fifteen-Year Trends in Incidence of Cardiogenic Shock Hospitalization and In-Hospital Mortality in the United States

Mohammed Osman et al.

Summary: The hospitalization rate attributed to cardiogenic shock tripled between 2004 and 2018, while there was a slow decline in in-hospital mortality associated with cardiogenic shock during the same period. Consistent reductions in mortality rates were observed across different demographics and hospital characteristics, indicating an overall positive trend in patient outcomes.
