Percutaneous transluminal septal myocardial ablation: past, present, and future

Summary: Approximately 30% of patients with hypertrophic cardiomyopathy have a significant left ventricular pressure gradient, with the majority diagnosed as hypertrophic obstructive cardiomyopathy. Percutaneous transluminal septal myocardial ablation has been introduced as a method to reduce this gradient and improve heart failure symptoms. However, the outcome is influenced by operators' experience and it is difficult to predict the response in advance. Advances in imaging modalities have helped in understanding the mechanisms of left ventricular outflow tract obstruction and will aid in better treatment selection.

Summary: This study aimed to investigate the prevalence and prognostic impact of atrial fibrillation (AF), particularly new-onset AF, in Japanese patients with hypertrophic cardiomyopathy (HCM). The presence of AF, especially new-onset AF, was associated with an increased risk of HCM-related adverse events, particularly heart failure events. AF may not only be a marker of disease stage but also an important trigger of adverse events in patients with HCM.
