Current laser therapy options for endoscopic treatment of upper tract urothelial carcinoma

Benjamin W. Zollinger et al.

Summary: Endoscopic management using retrograde ureteroscopic laser ablation is the preferred treatment for low-risk upper tract urothelial carcinoma (UTUC). Holmium:yttrium-aluminum-garnet (Ho:YAG) and neodymium (Nd:YAG) lasers have been commonly used over the past 15-20 years, but the thulium (Th:YAG) laser has recently emerged as a potential alternative. This review compares the mechanisms, physiological properties, effects, and oncologic outcomes of Ho:YAG/Nd:YAG lasers versus Th:YAG laser for UTUC treatment and discusses emerging laser technologies in UTUC management.

Retrograde intrarenal surgery: laser showdown (Ho:YAG vs thulium fiber laser)

Mariela Corrales et al.

Summary: This review evaluates the efficacy and safety of Ho:YAG laser and TFL in RIRS over the past two years. The findings suggest that TFL has a shorter laser-on time and operative time compared to Ho:YAG laser, while maintaining a similar level of safety. TFL is considered to be more efficient in performing RIRS.

Comparison of Holmium:YAG and Thulium Fiber Lasers on Soft Tissue: An Ex Vivo Study

Steeve Doizi et al.

Summary: This study evaluated the fiber-tissue interaction and degradation of laser fibers during soft tissue treatment using Ho:YAG laser and TFL. The results showed that Ho:YAG laser had greater incision depths and coagulation areas compared to TFL, but TFL had a constant carbonization zone. Additionally, degradation of laser fibers was observed with Ho:YAG laser, but not with TFL.

Summary: This study evaluated and compared the outcomes of ureterorenoscopic lithotripsy using Ho:YAG and TFL lasers. The results showed that TFL was superior to Ho:YAG in terms of stone clearance and reducing operative complications.

Effectiveness and Safety of Thulium Fiber Laser in the Conservative Management of Patients with Upper Tract Urothelial Carcinoma

Silvia Proietti et al.

Summary: This study retrospectively analyzed patients with UTUC who underwent conservative treatment using TFL and conducted a short-term follow-up. The results showed that TFL is a safe and effective technique that achieves optimal tumor ablation and hemostatic control without major complications.

Ureteroscopy with thulium fiber laser lithotripsy results in shorter operating times and large cost savings

James R. Ryan et al.

Summary: This retrospective study analyzed 152 cases of laser lithotripsy in ureteroscopy and found that Thulium fiber laser (TFL) lithotripsy has a significantly shorter operative time and lower cost compared to standard Holmium:YAG (Ho:YAG) lithotripsy, resulting in substantial savings in direct operating room costs.

Ex vivo study of Ho:YAG and thulium fiber lasers for soft tissue surgery: which laser for which case?

Mark Taratkin et al.

Summary: This study assessed the ablation, coagulation, and carbonization characteristics of the Ho:YAG laser and TFL. The results showed that the Ho:YAG laser had shallower cutting and minimal coagulation at lower power settings, while the TFL demonstrated effective cutting, prominent coagulation, and carbonization at all speeds. The study introduced TFL as an efficient alternative for soft tissue surgery to the Ho:YAG laser.

Thulium:YAGVersusHolmium:YAG Laser Effect on Upper Urinary Tract Soft Tissue: Evidence from anEx VivoExperimental Study

Silvia Proietti et al.

Summary: This study compared the effects of Holmium laser (Ho:YAG) and Thulium laser (Tm:YAG) on upper urinary tract soft tissues in an ex vivo experimental study. The results showed that Ho:YAG produced deeper incisions, while Tm:YAG led to larger coagulation areas and total laser areas. Future in vivo studies are needed to confirm these findings.
