Cognitive function after carotid endarterectomy in asymptomatic patients

Summary: Asymptomatic carotid stenosis has been linked to progressive neurocognitive decline, but the impact of carotid endarterectomy (CEA) is not well established due to varying studies and lack of standardization in testing and design. While there is growing evidence supporting the effectiveness of CEA in reversing or slowing neurocognitive decline, definitive conclusions are difficult to draw. Additional research is needed to clarify the relationship between asymptomatic carotid stenosis, the benefits of CEA, and protective effects against cognitive decline. This article aims to review current evidence on cognitive function in asymptomatic carotid stenosis patients undergoing CEA.

Routine versus selective near-infrared spectroscopy-guided shunting during carotid eversion endarterectomy

Summary: The aim of this study was to compare outcomes of routine shunting to near-infrared spectroscopy (NIRS)-guided shunting in patients undergoing eversion endarterectomy (EEA) under general anesthesia. The study found that, in patients undergoing EEA under general anesthesia, NIRS-guided selective shunting was associated with a lower incidence of perioperative stroke compared to routine shunting.

This narrative review provides an overview of the technological basis of these monitoring modalities and explores their application in predicting neurocognitive dysfunction and optimizing cerebral oxygen supply to reduce postoperative delirium and cognitive dysfunction following cardiac surgery.

Summary: This pilot study demonstrates the feasibility of noninvasively and in real-time monitoring cerebral hemodynamics during carotid endarterectomy using diffuse correlation spectroscopy (DCS) and near-infrared spectroscopy (NIRS). The results indicate that there is partial recovery of cerebral blood flow and an increase in blood volume during the surgery, with no significant changes in compensatory responses to different contralateral stenosis levels.
