Acute Pain Management Pearls: A Focused Review for the Hospital Clinician

Sara J. Hyland et al.

Summary: Acute pain management is a challenging area for inpatient clinicians, with a decrease in available specialists. This review provides clinical pearls and resources to help healthcare providers improve their knowledge and skills in acute pain management. It offers practical guidance for designing and adjusting multimodal analgesic regimens, including both opioid and non-opioid therapies. Customized care plans are emphasized for patients with preexisting opioid tolerance, chronic pain, or opioid use disorder, and recommendations for managing associated chronic therapies are discussed.

Analgesia for major laparoscopic abdominal surgery: a randomised feasibility trial using intrathecal morphine

K. Pirie et al.

Summary: Effective pain control is crucial for post-operative patient recovery. Intrathecal morphine is a popular analgesic option but its use in laparoscopic surgery lacks sufficient evidence. The feasibility trial conducted in this study supports the need for a definitive clinical trial.

The ALLEGRO trial: a placebo controlled randomised trial of intravenous lidocaine in accelerating gastrointestinal recovery after colorectal surgery

Hugh M. Paterson et al.

Summary: The ALLEGRO trial aims to assess the effectiveness of perioperative intravenous lidocaine in improving return of GI function after minimally invasive colorectal surgery. The study suggests that IV lidocaine can accelerate the recovery of GI function, reduce the rate of postoperative ileus, shorten hospital stay, alleviate pain and discomfort, and lead to faster recovery and discharge from hospital, resulting in significant cost savings for the National Health Service (NHS).

Transversus Abdominis Plane Block in Colorectal Surgery: A Meta-Analysis

Dmitriy Viderman et al.

Summary: The aim of this meta-analysis was to compare the postoperative opioid requirements of patients in the TAP block group and the control group in colorectal surgery. The results showed that the TAP block group had significantly lower opioid requirements and pain intensity at rest after laparoscopic and combined surgeries. However, TAP block did not have a significant impact on overall postoperative opioid consumption, length of hospital stay, and postoperative nausea and vomiting.

Current approaches to acute postoperative pain management after major abdominal surgery: a narrative review and future directions

Katrina Pirie et al.

Summary: Poorly controlled postoperative pain is associated with negative effects on patients' quality of life and functional recovery, including the risk of persistent pain and long-term opioid use. Alternative, opioid-minimising, multimodal analgesic strategies are needed in order to address opioid-related side-effects and the opioid abuse epidemic. Limited research exists on patient recovery quality after intraabdominal surgery using specific analgesic techniques.

Summary: This systematic review and meta-analysis found that in colorectal surgery, TAP block is equivalent to TEA in terms of postoperative pain control, but TAP block can provide better functional recovery and lower incidence of adverse events.

Perioperative Intravenous Lidocaine and Metastatic Cancer Recurrence - A Narrative Review

Thomas P. Wall et al.

Summary: Cancer is a major global health issue, with surgery offering potential cure for solid organ tumors, but with frequent postoperative recurrence. The choice of perioperative anesthetic and analgesic agents may influence cancer cell activity, with lidocaine showing potential effects in this regard.

The Routes of Administration for Acute Postoperative Pain Medication

Filomena Puntillo et al.

Summary: Effective treatment of postoperative acute pain, early mobilization, and nutrition are important perioperative strategies to improve surgical outcomes and reduce hospitalization costs. The choice of the most appropriate analgesics depends on drug class, route of administration, dosage, and individual patient limitations and contraindications. A comprehensive analysis of different routes of administration of acute postoperative pain medications helps in selecting suitable analgesics based on recent evidence and international recommendations.
