The Burden of Non-Alcoholic Fatty Liver Disease in Adolescents with Polycystic Ovary Syndrome: A Case-Control Study

Summary: The aim of this case-control study was to assess the burden of non-alcoholic fatty liver disease (NAFLD) in adolescents with polycystic ovary syndrome (PCOS) and its associations with insulin resistance, hyperandrogenism, and other metabolic characteristics of the syndrome. The study found that NAFLD was more prevalent in adolescents with PCOS compared to controls, and it was associated with insulin resistance and liver fat indices. Adolescents with PCOS and higher levels of free androgen index (FAI) had worse NAFLD and insulin sensitivity indices than those with lower FAI.

The close relationship between NAFLD and visceral obesity has led to the overshadowing of fatty liver as the main pathomechanism of insulin resistance and NCDs. However, the concept of hepatokines provides a new perspective on the role of the liver in metabolism and offers potential for precision medicine in NAFLD and visceral obesity.

The majority of studies focus on adult, premenopausal women and little attention has been given to young and adolescent women with NAFLD. Future research should include more diverse populations and study the contribution of excess androgens and insulin resistance to NAFLD risk in women with PCOS of varying weights, ages, and ethnicities.

Associations of Sex Steroids and Sex Hormone-Binding Globulin with Non-Alcoholic Fatty Liver Disease: A Population-Based Study and Meta-Analysis

Summary: This study investigated the associations between sex hormones and non-alcoholic fatty liver disease (NAFLD) and conducted a comprehensive systematic review and meta-analysis. The results showed that lower levels of sex hormone-binding globulin (SHBG) were associated with NAFLD in both sexes, while the association between testosterone and NAFLD varied by sex. In addition, estradiol may have a protective effect against NAFLD in healthy men.
