Diagnostics of Sacroiliac Joint Differentials to Axial Spondyloarthritis Changes by Magnetic Resonance Imaging

Summary: The diagnosis of axial spondyloarthritis (axSpA) is based on imaging and clinical findings due to the lack of diagnostic criteria. MRI of the sacroiliac joints (SIJ) has increased the frequency of axSpA diagnoses, especially in women, due to the presence of non-specific subchondral bone marrow edema (BME). Awareness of other differential diagnoses and understanding of relevant MRI and clinical features are important to accurately diagnose axSpA.

ASAS-EULAR recommendations for the management of axial spondyloarthritis: 2022 update

Summary: The ASAS-EULAR recommendations provide updated guidance on the management of axial spondyloarthritis, including treatment targets, non-pharmacological interventions, and the use of non-steroidal anti-inflammatory drugs (NSAIDs) as the first-line pharmacological treatment. For patients with comorbidities, the use of biological and targeted synthetic disease-modifying antirheumatic drugs (b/tsDMARDs) is recommended. The recommendations also cover the management of extramusculoskeletal manifestations, tapering of bDMARDs, and surgical interventions.

Summary: Magnetic resonance imaging (MRI) is critical for the early detection and follow-up of inflammatory rheumatologic disorders. Disease-specific knowledge and certain MRI parameters can help provide an early diagnosis and effective treatment. Awareness of hallmarks and consideration of age, sex, and history can prevent overdiagnosis. Whole-body MRI may be helpful in diagnosing specific disorders.

Diagnostic delay in axial spondyloarthritis: a systematic review

Summary: The diagnosis of axial spondyloarthritis (axSpA) is challenging, resulting in delayed diagnosis for patients. A systematic review found that the average diagnostic delay for axSpA ranged from 2 to 6 years, with a third of studies reporting a median delay of only 2 to 2.3 years. Factors such as gender and family history did not significantly influence the delay.

Summary: This study aimed to determine quantitative cut-offs for SI joint MRI lesions typical of inflammatory and structural lesions in axSpA, which can predict clinical diagnosis. The results showed that cut-offs for MRI findings specific to axSpA had PPVs greater than or equal to 95% for clinical diagnosis. Thus, the proposed cut-offs can be used for disease classification and clinical research to improve the diagnostic accuracy for axSpA.
