Summary: This study aimed to evaluate the safety and effectiveness of transvenous lead extraction (TLE) in octogenarians with a long lead dwell time. The results showed that the use of bidirectional rotational mechanical sheaths (Evolution RL) had a high success rate of 93.9%. However, the study also found that both 30-day and mid-term mortality rates were still high, especially in the presence of specific comorbidities.
