Tracking Klebsiella pneumoniae carbapenemase gene as an indicator of antimicrobial resistance dissemination from a hospital to surface water via a municipal wastewater treatment plant

Erica M. Loudermilk et al.

Summary: This study traced the presence of the antimicrobial resistance gene KPC and the viable organisms carrying this gene in hospital wastewater, municipal wastewater treatment plants (WWTP), and receiving water samples. The study found high concentrations of the KPC gene and the viable organisms in the hospital wastewater and the initial stages of the WWTP, but not in the final effluent following UV disinfection. The KPC-producing organisms were also found in the biosolids throughout the WWTP, but not in the processed compost-products made from the biosolids.

Genomic Diversity of Hospital-Acquired Infections Revealed through Prospective Whole-Genome Sequencing-Based Surveillance

Mustapha M. Mustapha et al.

Summary: Healthcare-associated infections (HAIs) can lead to mortality, morbidity, and wastage of healthcare resources. This study used whole-genome sequencing to analyze the diversity of bacteria sampled from hospitalized patients at a single center. The study identified nearly 100 different bacterial species and revealed different rates of evolution among species. Antibiotic resistance genes and mobile genetic elements were also frequently observed. This study provides valuable insights into the population structure and evolution of bacteria in healthcare settings and can inform efforts in outbreak detection and prevention.

Biofilm characteristics and transcriptomic profiling of Acinetobacter johnsonii defines signatures for planktonic and biofilm cells

Jia Jia et al.

Summary: Most bacteria in the environment form biofilms, which have inherent antibiotic tolerance. In this study, it was found that biofilm formation by Acinetobacter johnsonii is positively correlated with tetracycline resistance. Biofilms rapidly develop resistance to tetracycline when exposed to it. Persisters cells were found in both planktonic and biofilm cells, with a higher frequency in biofilms. Transcriptomic analysis revealed that planktonic cells overexpressed multidrug resistance and stress genes in response to sub-lethal concentrations of tetracycline, while biofilm cells showed lower metabolic levels, resulting in higher resistance to tetracycline. These findings suggest that A. johnsonii can adjust its phenotype and metabolism under antibiotic stress when growing as a biofilm, which has implications for biofilm control.

inStrain profiles population microdiversity from metagenomic data and sensitively detects shared microbial strains

Matthew R. Olm et al.

Summary: The program inStrain is used to study genetic diversity in microbial populations, particularly in fecal metagenomes of newborn premature infants. Results show that siblings share more microbial strains compared to unrelated infants, and infants born via cesarean section harbor bacteria with higher nucleotide diversity than vaginally delivered infants, potentially due to hospital acquisition. InStrain can be applied to analyze microdiversity and strain comparison in any metagenomic dataset.

Cheryl Volling et al.

Summary: Studies have found that sink drainage systems may be a reservoir for hospital-acquired infections, but the causality relationship needs further investigation. Some studies have provided evidence linking sink drainage systems with infections, interventions can effectively reduce the risk of bacterial infections, providing guidance for future research.

Approaches for characterizing and tracking hospital-associated multidrug-resistant bacteria

Kevin S. Blake et al.

Summary: Hospital-associated infections, especially those caused by antibiotic-resistant pathogens, remain a major concern for global public health. Despite extensive sanitization efforts, the hospital environment continues to serve as a significant reservoir and vector for antibiotic-resistant organisms. Advances in next-generation sequencing have provided new insights into the population dynamics of hospital-associated microbiota and the transmission networks of pathogens.

Genomic Investigation and Successful Containment of an Intermittent Common Source Outbreak of OXA-48-Producing Enterobacter cloacae Related to Hospital Shower Drains

Dennis Nurjadi et al.

Summary: The hospital water environment can serve as a reservoir for multiward outbreaks, leading to acquisitions or transmissions of multidrug-resistant organisms in a hospital setting. Most Gram-negative bacteria have the ability to form biofilms and persist in hospital plumbing systems for extended periods. Eliminating these reservoirs is crucial to preventing further transmission, but proposed decontamination regimens, such as using acetic acid, can only suppress rather than fully eliminate the environmental reservoir. This study demonstrated that colonization with multidrug-resistant organisms can be acquired through showering in showers with contaminated water traps and drains.
