The effect of systemic factors on retinal blood flow in patients with carotid stenosis: an optical coherence tomography angiography study

Lilla Istvan et al.

Summary: This study explored the impact of systemic risk factors on retinal blood flow in patients with significant carotid artery stenosis using OCT angiography. It revealed a negative effect of decreased estimated glomerular filtration rate (eGFR), hypertension, and carotid occlusion on retinal blood flow, while statin use and carotid surgery were shown to improve ocular microcirculation significantly. Other systemic factors, such as diabetes, clopidogrel or acetylsalicylic acid use, BMI, serum lipid level, and thrombocyte count, did not exhibit a significant effect on ocular blood flow.

Complicated Carotid Artery Plaques and Risk of Recurrent Ischemic Stroke or TIA

Anna Kopczak et al.

Summary: Complicated nonstenosing carotid artery plaques (CAPs) are an underestimated cause of stroke. This study aimed to determine the association between complicated CAP ipsilateral to acute ischemic anterior circulation stroke (icCAP) and recurrent ischemic stroke or transient ischemic attack (TIA). The study found that patients with icCAP had a higher frequency of recurrent events, and a ruptured fibrous cap and intraplaque hemorrhage were significantly associated with an increased risk of recurrence.

Changes of retinal structure and function in patients with internal carotid artery stenosis

Dong-hui Wu et al.

Summary: This study investigated the structural and functional changes of the retina in patients with different degrees of internal carotid artery (ICA) stenosis. The results revealed that patients with severe ICA stenosis had smaller central retinal artery equivalent (CRAE) and arteriole to venule ratio (AVR), as well as decreased retinal sensitivity. The retinal sensitivity was positively correlated with CRAE.

Retinal microvasculature and cerebral hemodynamics in patients with internal carotid artery stenosis

Junfeng Liu et al.

Summary: This study investigated the relationship between retinal microvasculature and cerebral hemodynamics in patients with internal carotid artery (ICA) stenosis. The results showed a strong correlation between retinal microvascular perfusion and cerebral perfusion in the superficial vascular layer, especially in patients with stenosis. Larger studies are needed to confirm these findings.

Associations among Stroke, Myocardial Infarction, and Amaurosis Fugax in a Tertiary Referral Hospital in Taiwan

Ming-Hui Sun et al.

Summary: This study found a high incidence of internal carotid artery stenosis after amaurosis fugax (AF) attack, which was significantly associated with stroke. The incidence of myocardial infarction (MI) and central retinal artery occlusion (CRAO) after AF was relatively low, and only CRAO was associated with carotid artery stenosis.

Risk of stroke in relation to degree of asymptomatic carotid stenosis: a population-based cohort study, systematic review, and meta-analysis

Dominic P. J. Howard et al.

Summary: The degree of asymptomatic carotid stenosis is positively associated with ipsilateral stroke risk, suggesting that the benefit of endarterectomy may be underestimated in patients with severe stenosis. Conversely, patients with moderate stenosis on contemporary medical treatment have a low 5-year stroke risk, calling into question the benefit of revascularization.

Imaging retinal microvascular manifestations of carotid artery disease in older adults: from diagnosis of ocular complications to understanding microvascular contributions to cognitive impairment

Lilla Istvan et al.

Summary: CAS is a consequence of systemic atherosclerotic disease affecting the aging populations of the Western world, often associated with cognitive impairment. Understanding the microvascular contributions to cognitive decline associated with CAS is crucial, and the retinal microvasculature provides a unique opportunity to study this. Ocular examination can be used as a noninvasive screening tool to investigate pathological changes in the CNS associated with CAS.
