Virological failure and adherence to antiretroviral therapy in adolescents and young adults living with human immunodeficiency virus

Summary: This systematic review aims to identify factors associated with virologic failure and adherence to drug therapy in adolescents and young adults living with HIV. The results showed that factors such as gender, alcohol use, low education, medication side effects, lack of social support, HIV/AIDS stigma, transportation barriers, and forgetfulness were linked to poor adherence to therapy. On the other hand, sufficient nutrition, good social support, confidence in therapy use, and fewer ART side effects were associated with good adherence. Additionally, factors such as low CD4 levels, alcohol use, substance abuse, low education, medication non-adherence, and forgetfulness were linked to virological failure.

Strategies to overcome HIV drug resistance-current and future perspectives

Summary: The availability of cART has greatly improved the course of HIV infection and the quality of life of patients, but drug resistant and multidrug resistant strains still pose a significant threat to treatment effectiveness. HIV drug resistance has been increasing exponentially in recent years, hindering efforts to end the HIV epidemic. New strategies for drug development focus on improving safety and resistance profiles, discovering drugs with novel mechanisms of action, and simplifying treatment. This review discusses the progress in salvage therapy for patients with multidrug-resistant HIV-1 infection and highlights the development of new antiretroviral agents and drug targets.

Transcriptome analysis of CD4+T cells from HIV-infected individuals receiving ART with LLV revealed novel transcription factors regulating HIV-1 promoter activity

Summary: Some HIV-infected individuals receiving antiretroviral therapy (ART) develop low-level viremia (LLV), which is associated with virologic failure. The characteristics of CD4+ T cells in LLV are largely unknown. In this study, we analyzed the transcriptome profiling of CD4+ T cells from healthy controls, HIV-infected patients with virologic suppression, and those with LLV. We found that CD4+ T cells in LLV expressed higher levels of Th1 signature transcription factors, toll-like receptors, anti-HIV entry chemokines, and antiIL-1& beta; factors compared to virologic suppression. We also identified pathways that could promote HIV-1 replication in LLV. Finally, we evaluated the effects of upregulated transcription factors on HIV-1 promoter activity and found that CXXC5 increased, while SOX5 suppressed HIV-1 transcription.

A Concentration Method for HIV Drug Resistance Testing in Low-Level Viremia Samples

Summary: This study assessed the performance of lentivirus concentration reagent (LCR) combined with Sanger sequencing (SS) in detecting drug resistance mutations (DRMs) in low-level HIV-1 viral load samples. The results showed that LCR can be an effective alternative method for monitoring DRMs in HIV-1 patients, especially in small- and medium-sized laboratories that cannot afford an ultracentrifuge.

Complicated genotypes circulating among treatment naive HIV-1 patients in Guangzhou, China

Summary: This study aimed to explore the characteristics of HIV-1 genotypes among treatment naive HIV/AIDS patients living in Guangzhou, China. Various CRFs and URFs were identified, with CRF07_BC, CRF01_AE, and CRF55_01B being the main circulating genotypes in Guangzhou. Analysis also revealed the presence of transmission networks among different risk populations, with a small percentage of patients showing pre-treatment drug resistance.
