Neutrophil extracellular traps: A novel target for the treatment of stroke

Ziyuan Zhao et al.

Summary: Stroke is a cerebrovascular disease caused by thrombus, with high morbidity and mortality rates. Neutrophils are the first cells to be recruited in the brain after stroke, and they aggravate brain injury through multiple mechanisms. Neutrophil extracellular traps (NETs) have been found to play important roles in the pathological process of stroke in recent years.

Projected Global Trends in Ischemic Stroke Incidence, Deaths and Disability-Adjusted Life Years From 2020 to 2030

Liyuan Pu et al.

Summary: We projected the global trends in ischemic stroke from 2020 to 2030 by age, sex, and socio-demographic index. The incidence rates of ischemic stroke were projected to increase across all age groups and socio-demographic index quintiles, with higher rates among women. However, the associated death and disability-adjusted life years rates were projected to decrease globally. The projections indicated a need for countries with a low socio-demographic index to be aware of potential increases in the age-standardized death and disability-adjusted life years due to ischemic stroke.

Enhanced Cerebroprotection of Xenon-Loaded Liposomes in Combination with rtPA Thrombolysis for Embolic Ischemic Stroke

Tao Peng et al.

Summary: This study demonstrates that the combination therapy of Xe-loaded liposomes and rtPA provides enhanced therapeutic efficacy in stroke treatment, leading to decreased neuronal cell death and mitigation of hemorrhagic side effects associated with late rtPA treatment.

Advances in Acute Ischemic Stroke Therapy

Yunyun Xiong et al.

Summary: The treatment of acute ischemic stroke is advancing with intravenous thrombolysis, mechanical thrombectomy, and cytoprotection being investigated. Later treatment with alteplase and the use of tenecteplase are beneficial alternatives to alteplase. Cytoprotection as an adjunct to reperfusion has shown no efficacy in prior clinical trials, but may have potential benefits.

The End of Tissue-Type Plasminogen Activator's Reign?

Imad Derraz

Summary: Mechanical thrombectomy is highly effective in treating acute ischemic stroke. The use of intravenous thrombolysis before mechanical thrombectomy remains controversial, but achieving complete reperfusion is the primary goal. However, it is unclear whether additional endovascular attempts or local lytics administration are needed.

Platelet-Neutrophil Association in NETs-Rich Areas in the Retrieved AIS Patient Thrombi

Ghulam Jeelani Pir et al.

Summary: The histological structure of thrombi plays a crucial role in the outcome of vascular recanalization therapy for AIS patients. This study investigated the composition and structure of thrombi in AIS patients, revealing that they were predominantly platelet-rich and contained complex platelet-rich areas associated with activated neutrophils and NETs. The NETs-containing areas showed strong reactivity to Willebrand factor, platelets, and HMGB1.

tPA Mobilizes Immune Cells That Exacerbate Hemorrhagic Transformation in Stroke

Kaibin Shi et al.

Summary: The study reveals that tPA has significant effects on immune cells in stroke patients and a rat model, leading to brain hemorrhage. Precision immune modulation can mitigate hemorrhagic transformation and improve neurological function post thrombolysis.

Cerebral endothelial cell-derived small extracellular vesicles enhance neurovascular function and neurological recovery in rat acute ischemic stroke models of mechanical thrombectomy and embolic stroke treatment with tPA

Chao Li et al.

Summary: The study demonstrates that small extracellular vesicles derived from rat cerebral endothelial cells (CEC-sEVs) in combination with tissue plasminogen activator (tPA) have a positive effect on treating cerebral large vessel occlusion, reducing infarct volume, increasing recanalization, improving cerebral blood flow, and reducing blood-brain barrier leakage. This treatment also effectively suppresses a network of microRNAs and proteins that mediate thrombosis, coagulation, and inflammation.
