International consensus on diagnosis and management of Dravet syndrome

Elaine C. Wirrell et al.

Summary: This study aimed to reach a consensus on the diagnosis and management of Dravet syndrome (DS) by consulting experienced physicians and caregivers. The study summarized the current literature and obtained opinions from an expert panel on the diagnosis and management of DS. The results showed that there was consensus on the diagnosis and treatment of DS, but there was disagreement on disease-modifying therapies.

Comparison of the effect of continuous intravenous infusion of sodium valproate and midazolam on management of status epilepticus in children

Ali Abbaskhanian et al.

Summary: The study compared the efficacy and side effects of sodium valproate and midazolam for pediatric status epilepticus, finding that sodium valproate was effective in 91.4% of patients and resulted in shorter seizure duration, while midazolam group had longer average stay in the hospital.

Early Exposure of Fosphenytoin, Levetiracetam, and Valproic Acid After High-Dose Intravenous Administration in Young Children With Benzodiazepine-Refractory Status Epilepticus

Abhishek G. Sathe et al.

Summary: This study investigated the drug concentrations of LEV and VPA in young children with status epilepticus, providing valuable information on plasma concentrations and protein binding. LEV concentrations ranged widely while VPA concentrations were after a 40 mg/kg dose. Challenges in patient enrollment and blood sampling were noted, highlighting the need for further studies with larger sample sizes to evaluate exposure-response relationships in this emergent condition.

Patterns of benzodiazepine underdosing in the Established Status Epilepticus Treatment Trial

Abhishek G. Sathe et al.

Summary: This study aimed to describe patterns of benzodiazepine use as first-line treatment of status epilepticus (SE) and found that benzodiazepines, particularly midazolam and lorazepam, are frequently underdosed throughout the United States. The study confirmed that underdosing is pervasive and difficult to remediate.

Benzodiazepine administration patterns before escalation to second-line medications in pediatric refractory convulsive status epilepticus

Theodore Sheehan et al.

Summary: This study evaluated benzodiazepine administration patterns in pediatric patients with refractory convulsive status epilepticus before transitioning to non-Benzodiazepine antiseizure medication (ASM). Findings include over one-third of patients receiving more than two benzodiazepines before transitioning, and delays in treatment initiation reducing the likelihood of multiple benzodiazepine doses. Patients with out-of-hospital seizure onset were more likely to receive additional benzodiazepines beyond 30 and 45 minutes, and intermittent SE was a risk factor for more benzodiazepines administered beyond 45 minutes.

A Phase 3 open-label study of the efficacy, safety and pharmacokinetics of buccally administered midazolam hydrochloride for the treatment of status epilepticus in pediatric Japanese subjects

Harumi Yoshinaga et al.

Summary: This study in Japan evaluated the efficacy of a midazolam oromucosal solution for treating pediatric patients with convulsive status epilepticus. Results showed that 80% of subjects achieved seizure termination within 10 minutes, with 84% experiencing seizure resolution within the same time frame. The study also demonstrated consistent safety and pharmacokinetic profiles with non-Japanese populations, suggesting that this alternative administration method may help reduce treatment time and minimize sequelae of prolonged seizures.

Efficacy of Home Anticonvulsant Administration for Second-Line Status Epilepticus Treatment

Rafael Wabl et al.

Summary: The study found that there was no significant difference in stopping seizures between patients who received a home medication and those who received a nonhome medication. However, retrospective evaluation suggested a potential association between receiving a nonhome medication and seizure cessation.
