Changes in cystic fibrosis transmembrane conductance regulator protein expression prior to and during elexacaftor-tezacaftor-ivacaftor therapy

Frauke Stanke et al.

Summary: Recently approved molecular therapeutics have shown improvement in CFTR protein expression and maturation. This study investigated the effects of triple therapy (ELX/TEZ/IVA) on CFTR protein expression and maturation in CF patients. The results showed that treatment with ELX/TEZ/IVA increased CFTR protein levels in 8 out of 12 patients, but the glycosylation of the mutant CFTR protein isoform C was still incomplete.

MDCT-based longitudinal automated airway and air trapping analysis in school-age children with mild cystic fibrosis lung disease

Oliver Weinheimer et al.

Summary: This study aimed to quantify airway dimensions and air trapping on chest CT scans of school-age children with mild CF-lung disease over two years, and found that there were regional interdependencies between wall thickening and air trapping, as well as short-term regional interdependencies between air trapping and an increase in bronchiectasis.

Spirometric and anthropometric improvements in response to elexacaftor/tezacaftor/ivacaftor depending on age and lung disease severity

Summary: Triple-combination therapy with ETI was introduced in Germany for CF patients. This study analyzed longitudinal data to identify the determinants of response to ETI in different age groups and severity levels. The results suggest that age and lung disease severity prior to ETI initiation play a role in the clinical response.

Standardized airway wall thickness Pi10 from routine CT scans of COPD patients as imaging biomarker for disease severity, lung function decline, and mortality

Kathrin Kahnert et al.

Summary: This study aimed to evaluate the association of Pi10, a measure of standardized airway wall thickness on CT, with exacerbations, mortality, and response to triple therapy in patients with COPD. The results showed that Pi10 was associated with COPD severity, comorbidities, lung function, respiratory therapy, and mortality, and it had some predictive ability.

Summary: The study demonstrates significant improvements in lung function, respiratory symptoms, and body mass index in CF patients using ETI therapy, regardless of their familiarity with modulators or prior drug use. Significant reductions in sweat chloride concentration were also observed, correlating with improved lung function in the overall study population.

Using chest computed tomography and unsupervised machine learning for predicting and evaluating response to lumacaftor-ivacaftor in people with cystic fibrosis

Summary: The study evaluated lung structural changes after 1 year of lumacaftor-ivacaftor treatment and identified morphological phenotypes associated with treatment response using unsupervised machine learning. The results showed significant improvement in bronchial disease on chest CT after 1-year treatment with lumacaftor-ivacaftor, and radiomics features on pre-therapeutic CT scans may help predict lung function response to the medication.

Artificial intelligence in computed tomography for quantifying lung changes in the era of CFTR modulators

Gael Dournes et al.

Summary: This study aimed to validate a fully automated artificial intelligence (AI)-driven scoring system of CF lung disease severity. The results showed that the AI-driven volumetric quantification had good correlations with visual imaging scoring and pulmonary function tests, providing a robust outcome for CF-related disease in the era of effective CF transmembrane conductance regulator modulator therapy.

CFTR Modulators in People with Cystic Fibrosis: Real-World Evidence in France

Lucile Regard et al.

Summary: Cystic fibrosis is a rare genetic multisystemic disease caused by mutations in the CFTR gene, and CFTR modulators have shown clinical efficacy in restoring CFTR protein function. However, questions remain regarding their long-term safety and effectiveness, especially in patients with advanced lung disease, liver disease, renal insufficiency, or problematic bacterial colonization. Further research is needed to investigate the impact of CFTR modulators on important outcomes such as concurrent treatments, lung transplantation, chest imaging, and pregnancies.

The Clinical Use of Lung MRI in Cystic Fibrosis What, Now, How?

Gael Dournes et al.

Summary: Recent technological breakthroughs and novel treatments have positioned lung MRI as a valuable, radiation-free tool for clinically assessing CF. With high-resolution imaging and the ability to differentiate between inflammation and scarring tissue, MRI can provide early guidance for treatment. Additionally, MRI can evaluate regional small-airway disease and offer automated quantification methods for more objective and reproducible assessment of disease severity.
