Efficacy of Replacing Linezolid with OTB-658 in Anti-Tuberculosis Regimens in Murine Models

Summary: Linezolid (LZD) is the first oxazolidinone approved for drug-resistant tuberculosis treatment. The BPaL regimen, combining LZD with bedaquiline (BDQ) and pretomanid (PMD), is an effective 6-month oral regimen against multidrug- and extensively drug-resistant tuberculosis. However, the toxicity of LZD due to mitochondrial protein synthesis inhibition may reduce its efficacy, and a safer and more effective alternative is needed. OTB-658, derived from LZD, shows better anti-TB activity and safety and could potentially replace LZD in TB treatment regimens.

Superior Efficacy of a TBI-166, Bedaquiline, and Pyrazinam de Combination Regimen in a Murine Model of Tuberculosis

Summary: TBI-166, derived from riminophenazine analogues, is a more potent anti-TB drug than clofazimine and is currently being tested in a phase IIa clinical trial in China. The results of this study demonstrate the superior efficacy of the TBI-166+BDQ+PZA regimen in treating murine TB models, compared to other regimens such as HRZ, TBI-166+BDQ+LZD, and BPaL. These findings suggest that TBI-166, in combination with BDQ and PZA, could be a promising treatment option for drug-resistant TB and should be further evaluated in phase IIb clinical trials.

Assessing Prolongation of the Corrected QT Interval with Bedaquiline and Delamanid Coadministration to Predict the Cardiac Safety of Simplified Dosing Regimens

Summary: This study investigated the relationship between plasma concentrations of delamanid and bedaquiline and the prolongation of QT interval corrected using Fridericia's formula (QTcF). The study found that the metabolites of each drug were responsible for the drug-related QTcF prolongation. The combination of the two drugs had a close to additive effect. The study also predicted that simplified, once-daily dosing regimens would have a similar QT prolonging potential compared to the approved regimens.
