Summary: This study analyzed the physiological handling of sodium burden induced by maintenance fluids and found that compared to a regimen resembling a healthy dietary sodium intake, especially in a clinical setting, sodium-rich maintenance fluids lead to prolonged fluid retention. The kidneys adapt to sodium-rich fluids by altering sodium excretion and concentrating urine to retain additional free water.

Interdisciplinary knowledge gaps on intravenous fluid management in adult patients: Survey among physicians and nurses of a university hospital

Summary: The study found that there is a lack of knowledge and understanding about intravenous fluids among physicians and nurses. They are not familiar with the composition of commonly prescribed IV solutions. Senior physicians do not perform better than junior physicians in terms of knowledge about the daily needs of patients who cannot eat. This highlights the need for structured information and continuing education to improve intravenous fluid management.

Restriction of Intravenous Fluid in ICU Patients with Septic Shock

Summary: According to a cross-sectional survey, there is a wide difference in the knowledge and prescription of intravenous fluids among healthcare professionals (HCP). The majority of respondents reported the absence of internal guidelines on IV fluids in hospitals or intensive care units. The average score on knowledge assessment questions was found to be low. This highlights the urgent need for education on the use of IV fluids.

Introducing NICE guidelines for intravenous fluid therapy into a district general hospital

Summary: This study aimed to assess the effects of implementing NICE guidelines on intravenous fluid prescribing in a general hospital setting. The study found that the use of 0.9% sodium chloride decreased by 75% and overall intravenous fluid use decreased. The incidence of acidosis decreased, and there were no adverse effects on other electrolytes. However, the incidence of stage 1 acute kidney injury increased. The study concludes that the implementation of the guidelines required significant effort and resource and recommends that prescribed volumes should still be carefully considered.

Observational study on fluid therapy management in surgical adult patients

Summary: The study investigated perioperative fluid therapy management in 131 Spanish hospitals, including 7291 patients. Balanced crystalloids were the most commonly used fluid, with colloids used in a small number of patients. Hourly surgical volume tended to be more restrictive in high-risk patients, but there was a high degree of variation in the overall volume administered perioperatively and scarce monitoring was observed.
