The Risk of Nonunion in Smokers Revisited: A Systematic Review and Meta-Analysis

Summary: The study aimed to review the impact of tobacco smoking on nonunion following spinal fusion through systematic review and meta-analysis. The results showed that smoking was associated with an increased risk of nonunion compared to not smoking for over a year following spine surgery. Smoking was significantly associated with increased nonunion in those receiving either allograft or autograft. Further studies with robust methodology are needed to establish treatment guidelines tailored to smokers.

Comparison of 3 treatment methods for midshaft clavicle fractures: A systematic review and network meta-analysis of randomized clinical trials

Summary: This study evaluated the impact of plate and screw fixation, intramedullary nailing, and non-surgical treatment on fracture nonunion rate and functional scores in midshaft clavicle fractures patients. The analysis of 24 randomized controlled trials involving 2161 participants showed that intramedullary nailing was superior to plate and screw fixation and non-surgical treatment in terms of reducing nonunion rate and improving functional scores.

Risk factors for complications and reoperation following operative management of displaced midshaft clavicle fractures

Summary: This study evaluated demographic factors, fracture pattern, and surgical technique as potential predictors of surgical complications following clavicle fracture open reduction internal fixation (ORIF). Smoking, obesity, and transverse or Z-type fractures were found to be significant prognostic factors for overall complication risk. Nonoperative management should be strongly considered in obese patients, smokers, and patients with transverse or Z-type fractures.

Displaced midshaft clavicle fractures in adults - is non-operative management enough?

Summary: The study found that non-operative treatment for displaced fractures of the middle third of the clavicle resulted in good functional outcomes after one year. Factors associated with non-union included smoking, diabetes, and poor soft tissue condition around the fracture. The most important factors influencing functional outcome were fracture angulation and clavicular shortening. Threshold values for these radiological displacements were determined to estimate the probability of achieving very good function.
