Summary: This study found that current standard diagnostic methods can easily miss cases of leptospirosis, and whole blood qPCR can be used as the leading standard test for diagnosing acute cases, while MAT should be limited to places with epidemiological interest. Additionally, qPCR testing of microscopically-negative culture samples should be done before discarding to increase the yield.

Pathogenic Leptospira Evolved a Unique Gene Family Comprised of Ricin B-Like Lectin Domain-Containing Cytotoxins

The findings suggest that LA3490 and other virulence-modifying proteins have evolved as novel bacterial toxins that combine cell surface binding, internalization, nuclear translocation, and DNase functions for cell killing.

Summary: The molecular and cellular pathogenesis of leptospirosis is still poorly understood. Recent studies have identified a gene family encoding secreted exotoxins that mediate cytotoxicity in vitro. Immunization with these exotoxins has been shown to prevent leptospirosis pathogenesis and reduce organ infection in an animal model. These findings support the potential use of exotoxin-based vaccines for leptospirosis.
