Longitudinal study of clinical and neurophysiological features in essential tremor

Luca Angelini et al.

Summary: This study investigated the progression of Essential Tremor (ET) manifestations through clinical and neurophysiological evaluation. The results showed that ET progression is characterized by the spread of tremor in multiple body segments and the emergence of soft signs. Female sex, absence of family history, and rest tremor at baseline were identified as predictive factors of worse disease progression.

Moving, fast and slow: behavioural insights into bradykinesia in Parkinson's disease

Damian M. Herz et al.

Summary: The cause of movement slowness in Parkinson's disease remains unclear, but it may be related to increased movement energy expenditure and difficulties in switching between stable and dynamic movement states. Understanding these abnormal behavioral computations is crucial for studying their underlying neural dynamics and for guiding future experimental research.

Short-term plasticity of the motor cortex compensates for bradykinesia in Parkinson's disease

Andrea Guerra et al.

Summary: Patients with Parkinson's disease show impaired short-term potentiation (STP) mechanisms in the primary motor cortex. In this study, the role of this neurophysiological abnormality in bradykinesia was investigated using a multimodal neuromodulation approach. The results suggest that abnormal STP processes are involved in bradykinesia pathophysiology and can be restored by increasing beta oscillations.

Parkinsonian Hand or Clinician's Eye? Finger Tap Bradykinesia Interrater Reliability for 21 Movement Disorder Experts

Summary: This study investigates the reliability of expert neurologists in assessing bradykinesia during the finger tapping test. The results show that even experts have considerable disagreement about the level of bradykinesia and frequently see bradykinesia in the hands of those without neurological disease.

Driving motor cortex oscillations modulates bradykinesia in Parkinson's disease

Andrea Guerra et al.

Summary: In this study, the authors found that abnormal beta and gamma oscillations in the basal ganglia are related to bradykinesia in patients with Parkinson's disease. Transcranial alternating current stimulation (tACS) can modulate cortical rhythms and was found to affect movement speed and amplitude in this study. Changes in inhibitory interneuronal activity may reflect compensatory mechanisms in the primary motor cortex (M1) to counteract bradykinesia.

The Movement Disorder Society Criteria for the Diagnosis of Multiple System Atrophy

Gregor K. Wenning et al.

Summary: This study aims to develop novel diagnostic criteria for multiple system atrophy (MSA) using an evidence-based and consensus-based methodology to improve the diagnostic accuracy, particularly in early disease stages. The criteria were developed and optimized through systematic literature review, Delphi rounds, and a virtual Consensus Conference. Different clinical diagnostic categories were defined, and a new concept of possible prodromal MSA was introduced. Brain magnetic resonance imaging markers were required for the diagnosis of clinically established MSA.

The Italian tremor Network (TITAN): rationale, design and preliminary findings

Roberto Erro et al.

Summary: The TITAN study is a multicenter data collection platform aimed at assessing the phenomenology and natural history of tremor syndromes. The study found that the frequency and characteristics of tremor syndromes vary. Patients with ET-plus were older and had a higher age at onset but a shorter disease duration compared to ET, suggesting that ET-plus is not a disease stage of ET.

Tremor and Movement Slowness Are Two Unrelated Adverse Effects Induced by Valproate Intake

Alessandro De Biase et al.

Summary: This study assessed the features of valproate-induced tremor (VIT) and the effect of valproate on the execution of voluntary movement in patients with epilepsy. The results showed that the severity of VIT worsened when valproate was taken in combination with other antiepileptics. Regardless of the presence of tremor, patients taking valproate performed finger tapping at a slower speed, similar to patients with Parkinson's disease. Patients with epilepsy who were not taking valproate showed normal motor performance.

Clinical and Kinematic Features of Valproate-Induced Tremor and Differences with Essential Tremor

Giulia Paparella et al.

Summary: This study conducted a clinical and kinematic assessment of 16 patients with valproate-induced tremor and 93 patients with essential tremor, revealing a higher occurrence of rest tremor and involvement of more body segments in valproate-induced tremor patients. The distinctive clinical and kinematic features of valproate-induced tremor suggest interference with cerebellar functions.

Bradykinesia in motoneuron diseases

Giulia Paparella et al.

Summary: The study found that ALS and PLS patients performed finger tapping slower than healthy controls, and the movement slowness correlated with muscle strength and amplitude of compound muscle action potential. The results highlight the pathophysiological role of motoneurons in generating movement slowness.

Mild Parkinsonian Signs: A Systematic Review of Clinical, Imaging, and Pathological Associations

Sarah M. Buchanan et al.

Summary: Mild parkinsonian signs (MPS) have been proposed as a risk marker for neurodegenerative disease, particularly Parkinson's disease (PD), in older adults. However, associations with MPS vary depending on age and other features of prodromal PD. Radiological findings for MPS are inconsistent, and pathological studies show mixed neuropathologies in older individuals with MPS. Future studies are needed to prospectively examine outcomes and associations of specific MPS subcategories.

Emerging concepts on bradykinesia in non-parkinsonian conditions

Giulia Paparella et al.

Summary: Bradykinesia is a common motor abnormality observed in various neurological diseases not primarily characterized by parkinsonism. The underlying pathophysiology of bradykinesia in these conditions may be related to dysfunction of brain networks. It is suggested that a more accurate and widely used definition of bradykinesia is needed in the context of movement disorders and other neurological conditions.
