Vestibular migraine or Meniere's disease: a diagnostic dilemma

Jing-Yu Chen et al.

Summary: Meniere's disease is a vertigo disorder characterized by recurrent vertigo, fluctuating hearing loss, tinnitus, or ear fullness. It is important to accurately differentiate Meniere's disease from vestibular migraine for effective treatment. Advances in examination technologies have improved the diagnostic accuracy of differentiating vertigo diseases.

Central positional nystagmus: an update

Joao Lemos et al.

Summary: Central positional nystagmus (CPN) can account for up to 12% of patients with positional nystagmus and is often associated with structural lesions in the cerebellum and/or brainstem. Treatment of CPN includes pharmacotherapy and causative treatment for underlying diseases.

Altered brain network functional connectivity patterns in patients with vestibular migraine diagnosed according to the diagnostic criteria of the Barany Society and the International Headache Society

Zhe-Yuan Li et al.

Summary: This study aimed to investigate altered patterns of brain network functional connectivity in patients with vestibular migraine (VM), and found significant changes in the sensorimotor network within VM patients, potentially leading to a hypersensitivity state. Additionally, adjustments in functional connectivity between the auditory network and default mode network, as well as salience network, may increase sensitivity to vestibular sensory processing in these patients.

Clinical characteristics of definite vestibular migraine diagnosed according to criteria jointly formulated by the Barany Society and the International Headache Society

Zhe-Yuan Li et al.

Summary: This study investigated the clinical features of patients with definite vestibular migraine (dVM). Among the 91 patients enrolled, 62 had dVM, with a higher prevalence in women. Vestibular symptoms were often accompanied by photophobia and phonophobia. Motion sickness and severe caloric test intolerance were helpful in diagnosing dVM. Some patients with dVM had abnormal immune-related indicators.

Exploratory saccades data analysis of video head impulse test in different Meniere's disease stages

Yi Du et al.

Summary: This study aimed to explore the nature and usability of saccades in different stages of Meniere's disease (MD). The results showed that the velocity of the first saccade on the affected side could indicate the disease progress and severity of MD, suggesting it as a potential new indicator.

Vestibular Migraine

Shin C. Beh

Summary: This review explores recent developments in vestibular migraine (VM), including the diagnostic criteria for VM, the wide range of vestibular symptoms and triggers, and the treatment options. The article highlights that many patients with VM experience persistent vestibular symptoms even in the absence of acute attacks, which greatly impacts their quality of life.

Dissociation of caloric and head impulse tests: a marker of Meniere's disease

I. P. Hannigan et al.

Summary: A retrospective analysis of 644 patients undergoing horizontal video head impulse test (vHIT) and caloric testing showed that in most cases, the results of the two tests were consistent, but there were 36 patients with inconsistent results.

Diagnostic assessment of patients with Meniere's disease through caloric testing and the video-head-impulse test

Livia Noleto de Rezende Oliveira et al.

Summary: Meniere's disease is a labyrinth disease characterized by vertigo, hearing loss, tinnitus, and aural fullness. Vestibular function tests may not be specific for diagnosis, but can help evaluate functional impairment. Results of caloric and video-head-impulse tests showed hyporeflexia in most patients with Meniere's disease, with more alterations in lateral semicircular canals.

Comparison of the results of caloric and video head impulse tests in patients with Meniere's disease and vestibular migraine

Mahmut Sinan Yilmaz et al.

Summary: The study found that caloric testing is more sensitive for detecting VOR pathology in patients with Meniere's disease compared to those with vestibular migraine, although video head impulse testing can provide complementary information. Abnormalities were more common in caloric testing for Meniere's disease, while abnormalities in video head impulse testing were more common in vestibular migraine patients.
