Immature Platelet Fraction Predicts Adverse Events in Patients With Acute Coronary Syndrome: the ISAR-REACT 5 Reticulated Platelet Substudy

Summary: This study aimed to evaluate immature or reticulated platelets as a predictive biomarker for adverse cardiovascular events in patients with acute coronary syndrome treated with potent P2Y(12) receptor inhibitors. The findings showed that immature platelets were significantly associated with the incidence of the primary endpoint, indicating their potential as a predictive marker for adverse cardiovascular events in these patients.

Role of Reticulated Platelets in Cardiovascular Disease

Summary: Reticulated platelets (RPs) are young, hyper-reactive platelets that are larger and contain more RNA compared to older platelets. They have higher thrombogenicity and are associated with insufficient antiplatelet response to aspirin and specific P2Y(12) receptor inhibitors. RPs are promising biomarkers for the prediction of adverse cardiovascular events in cardiovascular disease.

Summary: In this study, researchers found that neutrophils and platelets cooperate in the early stages of platelet formation. Neutrophils control platelet production by plucking intravascular platelet extensions. In steady state, neutrophils accelerate platelet growth and facilitate continuous release. However, after myocardial infarction, excessive release of young platelets driven by neutrophil plucking increases the risk of recurrent ischemia. Ablation of neutrophil plucking reduces thromboischemic events and thrombus burden.
