Thoracoabdominal normothermic regional perfusion in donation after circulatory death does not restore brain blood flow

Jennifer A. Frontera et al.

Summary: The use of thoracoabdominal normothermic regional perfusion (TA-NRP) in donation after circulatory death (DCD) is an important development in organ donation. However, there have been concerns about the restoration of brain blood flow. In this study, the authors used intraoperative transcranial Doppler (TCD) to assess brain blood flow in two DCD TA-NRP cases and found no evidence of restored blood flow.

Summary: The American Society of Transplant Surgeons supports increasing organ availability for transplantation. Variability in the utilization and practices of donation after circulatory death (DCD) transplantation exists. To improve DCD organ utilization, a work group has proposed recommendations on controversial or lacking standardized topics in DCD procurement.

The role of sequential normothermic regional perfusion and end-ischemic normothermic machine perfusion in liver transplantation from very extended uncontrolled donation after cardiocirculatory death

Davide Ghinolfi et al.

Summary: The use of pre-procurement normothermic regional perfusion (NRP) allows for controlled DCD liver transplantation, with comparable results to brain death donors. However, logistic challenges and post-transplant complications have led to a decline in the use of uncontrolled DCD. In Italy, the combination of NRP and ex-situ machine perfusion has been used to counterbalance the negative effects of prolonged warm ischemia.

Disparities in the Use of Older Donation After Circulatory Death Liver Allografts in the United States Versus the United Kingdom

Emmanouil Giorgakis et al.

Summary: There are differences between the United States and the United Kingdom in terms of characteristics and posttransplant survival of patients who received donation after circulatory death (DCD) liver allografts from donors aged >60 years. The utilization of older DCD liver transplants is low in the United States, resulting in worse survival for donors aged >60. However, survival rates for donors aged <= 60 are similar in both countries, supporting an extension to the current US DCD age cutoff.

Textbook Outcome as a Quality Metric in Liver Transplantation

Austin D. Schenk et al.

Summary: Traditionally, the quality of liver transplantation is measured by 1-year patient and graft survival rates. However, as these survival rates exceed 90%, there is a need for more informative metrics. Textbook outcomes (TOs) describe ideal patient outcomes after surgery, and this study shows that TO is an important measure of quality in liver transplantation, in addition to 1-year patient and graft survival rates.

Evolving utilization of donation after circulatory death livers in liver transplantation: The day of DCD has come

Summary: Increased clinical experience with DCD LT has led to higher utilization rates of livers from DCD donors, shorter ischemic and hospitalization times, and lower rates of retransplantation. Over the past decade, the rate of DCD LTs conducted in the United States has outpaced that of DBD, indicating an opportunity to capitalize on underutilized DCD liver allografts to mitigate the organ shortage in liver transplantation.

Recommendations for Donor and Recipient Selection and Risk Prediction: Working Group Report From the ILTS Consensus Conference in DCD Liver Transplantation

Summary: Despite the increasing utilization of DCDs for LT, there is still controversy and no universal acceptance criteria for donor and recipient risk factors and prediction models. The International Liver Transplantation Society organized a consensus conference in Venice, Italy on January 31, 2020, to review current clinical practices worldwide and develop internationally accepted guidelines. Experts in the field were divided into 6 working groups to provide evidence-based recommendations, which were then reviewed and confirmed by a jury of conference attendees. Working group 2 presented final statements and recommendations covering donor and recipient risk factors and prediction models in DCD-LT.

Summary: DCD grafts are commonly used in liver transplantation, but carry increased risk for ischemia/reperfusion injury and postoperative complications due to the additional ischemic event during donor warm ischemia time. Vital parameters after withdrawal of life support are crucial for successful transplantation, and the duration of functional DWIT is a key factor to consider.
