Expanding the Role of Continuous Glucose Monitoring in Modern Diabetes Care Beyond Type 1 Disease

Tomasz Klupa et al.

Summary: The application of continuous glucose monitoring (CGM) has revolutionized diabetes care by shifting from a reactive to proactive approach. CGM devices are now considered the gold standard for people with type 1 diabetes and evidence supports their use in type 2 diabetes as well. Expanding the use of CGM can lead to improved therapy, reduced complications, and lowered healthcare costs, all while enhancing quality of life for individuals with diabetes. Additionally, CGM can provide benefits for pregnant women with diabetes, non-diabetic individuals with glycemic dysregulation, and hospital inpatients.

Should continuous glucose monitoring be used to manage neonates at risk of hypoglycaemia?

Maria-Sofia Kalogeropoulou et al.

Summary: The National Institute for Clinical Excellence (NICE) recommends continuous glucose monitoring (CGM) for adults and children with diabetes at risk of hypoglycemia. CGM can detect silent hypoglycemia in neonates, which is associated with poor neurodevelopmental outcome. Despite advancements in technology, adoption in clinical practice is limited due to device limitations for newborns. However, CGM offers the opportunity to improve understanding of hypoglycemia risks and the impact of interventions.

Feasibility of intraoperative continuous glucose monitoring: An observational study in general surgery patients

Catherine E. Price et al.

Summary: Perioperative hyperglycemia is associated with adverse outcomes in surgical patients, and continuous glucose monitors (CGMs) have not been widely used in this population. This study evaluated the use of CGMs compared to standard practices in surgical patients with diabetes. CGMs provided more glycemic data and could potentially improve perioperative glycemic control.

Mixed Meal Tolerance Test Versus Continuous Glucose Monitoring for an Effective Diagnosis of Persistent Post-Bariatric Hypoglycemia

Ana M. Ramos-Levi et al.

Summary: Gastric bypass surgery can lead to increased incretin secretion and glucose excursions, potentially causing post-bariatric hypoglycemia (PBH). This study evaluated the effectiveness of mixed meal tolerance tests (MMTT) and continuous glucose monitoring (CGM) for diagnosing PBH. The MMTT confirmed PBH diagnosis in a large percentage of patients, especially those with longer postsurgical duration, while CGM identified both nocturnal asymptomatic and daytime postprandial hypoglycemia. The results highlight the importance of both tests in diagnosing and monitoring PBH after gastric bypass.

Summary: This review summarizes the use of continuous glucose monitoring devices in the hospital setting. CGM provides real-time glucose data that enable healthcare professionals to make proactive and timelier clinical decisions. Early studies show that CGM use in the hospital is well received by nurses.

A Comparison of the Blood Glucose, Growth Hormone, and Cortisol Responses to Two Doses of Insulin (0.15 U/kg vs. 0.10 U/kg) in the Insulin Tolerance Test: A Single-Centre Audit of 174 Cases

Phillip Yeoh et al.

Summary: This study compared the effects of a standard dose and a low dose of insulin tolerance test (ITT) on glucose, cortisol, and growth hormone (GH) responses. The results showed that the low-dose ITT produced comparable glucose, cortisol, and GH responses to the standard dose. Given the risks associated with hypoglycaemia, the low dose appears to be preferable to the standard dose ITT in most circumstances.

Summary: The HPA axis is essential for maintaining normal physiological events and survival, with cortisol playing a key role. While a correct diagnosis of AI can often be made with basal cortisol levels, more complicated tests may be necessary for SAI cases. Numerous studies have been published on assessing the HPA axis, with detailed discussions and recommendations for diagnosing secondary AI or investigating suspected HPA axis insufficiency.
