Advances in the management of Acanthamoeba keratitis: A review of the literature and synthesized algorithmic approach

Summary: AK is a severe cause of infectious keratitis that poses a significant clinical challenge. This review article evaluates recent literature on AK epidemiology, diagnosis, treatment, and prognosis, and proposes an algorithmic protocol for managing AK. The global incidence of AK outbreaks is increasing, especially in developed countries. Despite efforts to recall contact lens solutions associated with AK outbreaks, the problem persists. New diagnostic technologies, such as in vivo confocal microscopy and PCR assays, have improved the ability to diagnose AK. The management of AK typically involves the use of biguanides and diamidines, however, other treatment modalities, such as voriconazole and miltefosine, are also being explored. Benzalkonium chloride preservative and Rose-Bengal photodynamic antimicrobial therapy may have potential therapeutic uses. Topical corticosteroids have been shown to worsen AK outcomes. Overall, further research is needed to optimize the treatment of AK.
