Spread of local anesthetics after erector spinae plane block: an MRI study in healthy volunteers

Summary: The study found that after ESPB, local anesthetic consistently spread to the erector spinae muscles, the intercostal space, and the neural foramina. Epidural spread was evident in some volunteers. Sensory testing 30-50 minutes after ESPB showed highly variable results, generally under-representing the observed spread of the anesthetic on MRI.

Summary: Peripheral nerves are protected by a blood-nerve barrier that prevents harmful substances and pathogens from invading, while also regulating metabolism and ion balance within nerve fascicles. The barrier is formed by the concentric layer of perineurial cells and blood vessels in the endoneurium. This barrier allows glucose to be taken up as the main nutrient by the perineurium and endoneurial capillaries, while other nutrients are transferred by the perineurial cells only. The importance of the nerve sheaths in the development, maintenance, and diseases of peripheral nerves is increasingly recognized.

Mechanisms of action of the erector spinae plane (ESP) block: a narrative review

Anatomy of the thoracic paravertebral space: 3D micro-CT findings and their clinical implications for nerve blockade
