Hepatitis C Virus Infection Preceding an Outbreak of Human Immunodeficiency Virus Among Persons Who Inject Drugs-Kanawha County, West Virginia, 2019-2021

Alana G. Hudson et al.

Summary: Among 65 cases of HIV outbreak among persons who inject drugs (PWID) in West Virginia (2019-2021), 94% were diagnosed with hepatitis C a median of 46 months before HIV diagnosis. Diagnosis of hepatitis C among PWID should lead to improved access to prevention and treatment services.

Hepatitis C coinfection and extrahepatic cancer incidence among people living with HIV

Sarah J. Willis et al.

Summary: This study assessed the incidence of extrahepatic cancer among people with HIV/HCV coinfection and found that the risk of any extrahepatic cancer among patients with untreated HCV coinfection at baseline was similar to the risk if they had been successfully treated for HCV, but not statistically significant. Treating HCV coinfection with DAAs did not show evidence of reducing the incidence of extrahepatic cancers among people with HIV receiving ART.

Summary: This study estimated trends in the national prevalence of HCV infection in the US civilian population, finding that while the prevalence of active HCV infection remained stable, the prevalence of resolved HCV infection increased after the introduction of DAA. In terms of health insurance coverage, individuals with health insurance, especially private insurance, saw a decrease in active HCV infection and an increase in resolved HCV infection.

Summary: The study found that factors such as age, insurance status, CD4 cell count, history of drug use, and care location can influence whether HIV/HCV coinfected individuals in routine HIV care are prescribed DAAs for HCV treatment. Less than half of HIV/HCV coinfected patients in HOPS have been prescribed DAAs. Intervention is needed to address deficits in DAA prescription, particularly among patients with public or no health insurance, younger age, and lower CD4 cell count.

Syringe Services Programs' Role in Ending the HIV Epidemic in the US: Why We Cannot Do It Without Them

Summary: Diagnoses of HIV among people who inject drugs have increased in the U.S. for the first time in 2 decades, with multiple HIV outbreaks detected since 2015. Syringe services programs are cost effective and highly effective in reducing HIV transmission but have limited geographic coverage. Combining syringe services programs with medications for substance use disorder and antiretroviral therapy can maximize their impact in HIV prevention.

Summary: In 2018, a geographically concentrated outbreak of HIV among heterosexual people who inject drugs was identified in King County, Washington. The outbreak involved a cluster in North Seattle with 30 people diagnosed with related HIV infections. Intervention activities included a rapid needs assessment, HIV screening tests, provision of clean syringes, and public communications. Earlier identification of the outbreak could have been achieved with a lower cluster size threshold and more rapid analyses of data.
