Emerging epigenetic therapies of cardiac fibrosis and remodelling in heart failure: from basic mechanisms to early clinical development

Timothy A. McKinsey et al.

Summary: Cardiovascular diseases and heart failure have a significant impact on global health and impose a large economic burden. Despite advances in standard care, the risks for death and readmission of heart failure patients are still unacceptably high, necessitating the development of new therapeutic strategies. This review summarizes current knowledge on novel cellular and epigenetic therapies, with a focus on non-coding RNA-based strategies in early clinical development to counteract cardiac remodeling and heart failure. The review also discusses the current status, limitations, and future perspectives of translating these novel therapies into clinical practice.

Summary: Schisandra chinensis and Schisandra sphenanthera, known as bei-wuweizi and nan-wuweizi in Chinese respectively, are highly similar plants with traditional uses in Chinese medicine for treating various conditions. They have similar chemical compositions and pharmacological activities, with potential drug interactions due to their inhibitory effects on CYP3A and P-gp. Further research is needed to explore their traditional uses and differences for clinical applications.

Current and emerging drug targets in heart failure treatment

Nicolo Ghionzoli et al.

Summary: The neurohormonal interpretative model has laid the foundation for current pharmacological management of heart failure, with drugs targeting detrimental neurohormonal axes and newer approaches focusing on improving cardiac function and remodeling. Emerging therapies are shifting the therapeutic target from neurohormonal systems to the heart, aiming to enhance cardiac contractility, metabolism, fibrosis, inflammation, and remodeling.

Targeting TGF-β signal transduction for fibrosis and cancer therapy

Dandan Peng et al.

Summary: TGF-beta plays essential roles in early embryonic development, organogenesis, immune regulation, tissue repair, and adult homeostasis. Its involvement in fibrosis and cancer is complex and context-dependent, showing both inhibitory and promoting effects. Therapeutics targeting TGF-beta signaling hold promise in treating fibrosis and cancer, although the development is hindered by potential systemic cytotoxicity.

Bicyclol Attenuates Obesity-Induced Cardiomyopathy via Inhibiting NF-κB and MAPK Signaling Pathways

Yanghao Chen et al.

Summary: The study aimed to investigate the effect and mechanism of bicyclol on obesity-induced cardiomyopathy. The results demonstrated that bicyclol significantly alleviated myocardial inflammation, fibrosis, and hypertrophy caused by a high-fat diet and palmitic acid stimulation. This effect was achieved by inhibiting the MAPK and NF-kappa B pathways.

Cardiac fibrosis

Nikolaos G. Frangogiannis

Summary: Myocardial fibrosis, characterized by the expansion of cardiac interstitium through deposition of extracellular matrix proteins, is a common pathophysiologic feature in various myocardial conditions. Activated fibroblasts and myofibroblasts play a central role in cardiac fibrosis, producing matrix proteins and triggering fibrogenic signalling cascades in response to stress. Immune cells, vascular cells, and cardiomyocytes can also contribute to fibrosis, while fibrotic changes may disrupt cardiac function and play a role in arrhythmogenesis.
