Changes in cerebral autoregulation and vasoreactivity after surgical aortic valve replacement: a prospective study

Summary: This study found that dynamic cerebral autoregulation is preserved in patients with severe aortic stenosis in the long term, and does not change after surgical aortic valve replacement. However, carbon dioxide vasoreactivity is impaired in these patients. The study suggests that severe aortic stenosis does not affect dynamic cerebral autoregulation, but reduces vasoreactivity to carbon dioxide. Surgical aortic valve replacement does not alter cerebral autoregulation or vasoreactivity.

Changes of the cardiac baroreflex bandwidth during postural challenges

Summary: This study proposes a model-based parametric approach for estimating the baroreflex bandwidth, which provides different information compared to baroreflex sensitivity. The method takes into account the action of mechanisms changing heart rate irrespective of systolic arterial pressure. The study found that the baroreflex bandwidth changes with different degrees of head-up tilt.

Model-based spectral causality of cardiovascular variability interactions during head-down tilt

Summary: This study investigates the variability between heart rate (HP) and systolic arterial pressure (SAP) in healthy men during head-down tilt (HDT) using model-based spectral causality analysis. The findings suggest that HDT reduces the involvement of the baroreflex in regulating the relationship between HP and SAP in the low frequency band, but does not affect the action of mechanical feedforward mechanisms in both low and high frequency bands.

Concomitant evaluation of cardiovascular and cerebrovascular controls via Geweke spectral causality to assess the propensity to postural syncope

Summary: The evaluation of propensity to postural syncope requires a method that can characterize cardiovascular and cerebrovascular controls and analyze causal relationships in the frequency domain. We used Geweke spectral causality (GSC) to assess cardiovascular control and cerebrovascular regulation in control subjects and individuals prone to orthostatic syncope. We found that GSC markers were independent of the model structure, and concomitant assessment of both controls provided a deeper understanding of postural disturbances. Orthostatic syncope appeared to be influenced by a loss of coordination between baroreflex feedback and mechanical feedforward pathways, as well as weak cerebral autoregulation. GSC is a promising tool for describing and monitoring disturbances associated with posture modification.

Dynamic cerebrovascular autoregulation in patients prone to postural syncope: Comparison of techniques assessing the autoregulation index from spontaneous variability series

Summary: Three approaches to assess cerebrovascular autoregulation were applied, using the autoregulation index computed from the variability of mean arterial pressure and mean cerebral blood flow velocity. Different methods provided different results, but none indicated significant differences in the dynamic component of cerebrovascular autoregulation between individuals with and without a history of postural syncope.

Monitoring the Evolution of Asynchrony between Mean Arterial Pressure and Mean Cerebral Blood Flow via Cross-Entropy Methods

Summary: This study explored the use of two nonlinear tools in the information domain to assess the asynchrony between mean arterial pressure (MAP) and mean cerebral blood flow (MCBF). The results showed that the degree of asynchrony increased at rest and decreased after surgical aortic valve replacement (SAVR). This has important implications for the noninvasive evaluation of cerebrovascular control.

An Integrated Multimodal Approach to Evaluate Autonomic Control, Cerebral Autoregulation and Cognitive Function in Patients Undergoing Surgical Aortic Valve Replacement during a 3-Months Follow-up

Summary: This study proposes a multimodal approach to evaluate the impact of surgical aortic valve replacement (SAVR) on cardiovascular (CV) and cerebrovascular (CBV) control. Results showed that CV control was affected in the week after surgery, but recovered after three months. Diffusion-weighted magnetic resonance imaging (DW-MRI) indicated the presence of ischemic lesions in a high percentage of patients, but their cognitive function and CBV control remained preserved at all time points. A multimodal approach can accurately monitor postoperative recovery and identify high-risk patients for post-surgery CBV adverse events.
