Pacing-induced cardiomyopathy: A systematic review and meta-analysis of definition, prevalence, risk factors, and management

Summary: Pacing-induced cardiomyopathy (PiCM) is a potential complication of right ventricular pacing, and its definition varies among studies. This systematic review and meta-analysis evaluated the definition, prevalence, risk factors, and treatment strategies of PiCM. The pooled prevalence of PiCM was 12%, and risk factors included male sex, history of myocardial infarction, chronic kidney disease, atrial fibrillation, and various cardiac parameters. Treatment strategies included biventricular cardiac resynchronization therapy and His-bundle pacing.

This strategy seems to have similar effects on left ventricular ejection fraction (LVEF) as de-novo CRT, but more evidence on procedure-related complications is needed from prospective studies or randomized controlled trials.
