SHPT is largely managed through diet, ultraviolet exposure, vitamin D intake, and dialysis. Drugs and surgery are largely held in reserve for cases of refractory PTH elevation, complications of CKD and comorbid conditions, and in preparation for planned renal transplantation. A team approach to managing these patients is essential. Guidelines are reviewed in Box 4.
