Normocalcaemic primary hyperparathyroidism: is nephrolithiasis more common than osteoporosis?

Kevin Cho Jung Wu et al.

Summary: This study aims to determine the prevalence and clinical significance of normocalcaemic primary hyperparathyroidism (NPHPT) in an unselected sample in an acute hospital setting. The findings of this study provide further support for the diagnosis of NPHPT as a clinical entity. Nephrolithiasis may be a more prevalent issue than osteoporosis in patients with NPHPT compared to primary hyperparathyroidism (PHPT). Prospective evaluation is needed to confirm these findings.

Phenotypes of primary hyperparathyroidism: Does parathyroidectomy improve clinical outcomes for all?

Valerie L. Armstrong et al.

Summary: This retrospective study found that patients with normocalcemic primary hyperparathyroidism and normohormonal primary hyperparathyroidism have a higher risk of disease persistence and postoperative nephrolithiasis, but they also demonstrate improvements in postoperative bone density. These findings can inform preoperative discussions with patients with these phenotypes of primary hyperparathyroidism regarding postoperative expectations.

What happens to the bone structure after normocalcemic primary hyperparathyroidism surgery?

Irene Osorio-Silla et al.

Summary: This study evaluated the bone changes in patients undergoing parathyroidectomy. The results showed that surgery had similar benefits for normocalcemic primary hyperparathyroidism and hypercalcemic primary hyperparathyroidism in terms of bone improvement.

Evaluation and Management of Primary Hyperparathyroidism: Summary Statement and Guidelines from the Fifth International Workshop

John P. Bilezikian et al.

Summary: The last international guidelines on the evaluation and management of primary hyperparathyroidism (PHPT) were published in 2014. New research since then has led to new insights in epidemiology, pathophysiology, diagnosis, measurements, genetics, outcomes, presentations, new imaging modalities, target and other organ systems, pregnancy, evaluation, and management. An international group of experts reviewed these advances in knowledge and provided evidence-based recommendations and guidelines for the evaluation and management of PHPT.

Comparisons Between Normocalcemic Primary Hyperparathyroidism and Typical Primary Hyperparathyroidism

Hye Ryeon Choi et al.

Summary: Normocalcemic primary hyperparathyroidism (NPHPT) is a heterogeneous disease entity of primary hyperparathyroidism, characterized by high rates of multi-gland disease and mild biochemical abnormalities but symptomatic presentation. Intraoperative PTH monitoring may improve surgical outcomes, and short-term outcomes after surgery are similar to those of typical primary hyperparathyroidism (TPHPT).

A practical approach to normocalcemic primary hyperparathyroidism

Jose Luis Munoz de Nova et al.

Summary: Normocalcemic primary hyperparathyroidism poses challenges in clinical management due to unclear guidelines with no evidence-based specific recommendations. A comprehensive approach is necessary, considering diagnosis, differential diagnosis, treatment indications, surgical options, and medical treatment for patients who are not candidates for surgery.

Summary: NHPT is a milder form of classical primary hyperparathyroidism, characterized by frequent oscillations of serum calcium challenging the diagnosis. It may be a cardiovascular risk factor.
