Photon counting Detectors: Concept, technical Challenges, and clinical outlook

Bjoern Kreisler

Summary: Photon counting detectors in Computed Tomography have many advantages over traditional detectors, as they can individually detect incoming photons, resulting in higher contrast and resolution, lower doses, and more detailed images with additional energy information.

Long-Term Clinical Experience with Directional Deep Brain Stimulation Programming: A Retrospective Review

Jessica A. Karl et al.

Summary: The study examines the implementation of directional deep brain stimulation (d-DBS) in clinical practice, finding that 39-68% of patients with Parkinson's disease and 50-72% of patients with essential tremor had at least one lead programmed directionally over a 36-month period. This approach aims to improve symptom control and reduce side effects.

Longitudinal Assessment of Rotation Angles after Implantation of Directional Deep Brain Stimulation Leads

Florian Lange et al.

Summary: The risk/benefit-ratio of deep brain stimulation (DBS) depends on focusing the electrical field onto the target volume, excluding side-effect eliciting structures. Directional leads limiting radial current diffusion can target stimulation but add a spatial degree of freedom that requires control to align multimodal imaging datasets and for anatomical interpretation of stimulation. Results from a retrospective analysis of clinically indicated, repeated neuroimaging controls postimplantation show that rotation angles deviate significantly from their intended orientation, but can be measured accurately using both CCT and rotation fluoroscopies methods. Rotation of directional leads is established within the first postoperative day and can be safely initiated for directional stimulation settings without risking subsequent anatomical shifts.

Temporal Stability of Lead Orientation in Directional Deep Brain Stimulation

Till A. Dembek et al.

Summary: The study aimed to determine if directional leads remain in a fixed orientation after implantation or whether orientation changes over time. The results showed that the orientation of directional leads was stable with no clinically relevant changes either in the first weeks after implantation or over longer periods of time. Multiple CT scans were used to confirm the constancy of the orientation angle in patients.

Reduced Programming Time and Strong Symptom Control Even in Chronic Course Through Imaging-Based DBS Programming

Florian Lange et al.

Summary: The study found that image-guided DBS programming can significantly reduce programming time in Parkinson's disease patients while maintaining similar levels of symptom control. Although both programming methods had similar clinical outcomes, image-guided programming was faster in time duration.

DiODe v2: Unambiguous and Fully-Automated Detection of Directional DBS Lead Orientation

Till A. Dembek et al.

Summary: Directional deep brain stimulation (DBS) leads are widely used, but determining their orientation accurately from postoperative CT scans is crucial. An enhanced algorithm based on center of mass shifts of directional markers compared to their expected geometric center was found to be the most reliable method for resolving orientation ambiguity in DBS leads. This DiODe v2 algorithm offers an automated solution for determining the orientation of directional DBS leads.

Directional Stimulation in Parkinson's Disease and Essential Tremor: The Cleveland Clinic Experience

Richard A. Rammo et al.

Summary: The study found that directional stimulation was more commonly used than omnidirectional stimulation in patients with Parkinson's disease and essential tremor. Significant improvements in therapeutic current strength, therapeutic window, and total electrical energy were found in leads programmed with directional stimulation.

Directional Deep Brain Stimulation for Parkinson's Disease: Results of an International Crossover Study With Randomized, Double-Blind Primary Endpoint

Summary: Published reports on deep brain stimulation for Parkinson's disease have been limited to small studies, but this study found that directional stimulation resulted in a wider therapeutic window compared to omnidirectional stimulation, and was preferred by both subjects and clinicians involved in the study.
