Clinical characteristics and clinical course of myelin oligodendrocyte glycoprotein antibody-seropositive pediatric optic neuritis

Summary: The purpose of this study was to investigate the ophthalmic and neurological features of myelin oligodendrocyte glycoprotein antibody seropositive optic neuritis (MOG-ON) in pediatric patients. The results showed that pediatric MOG-ON has distinct clinical features, such as infrequent pain and perineural enhancement. Although optic atrophy is commonly observed, visual function is retained in most patients. Therefore, a multidisciplinary approach and long-term follow-up are required for pediatric MOG-ON, as CNS involvement is more common than ON recurrence.

Summary: This cohort study suggests that onset age and onset phenotype should be considered when assessing subsequent relapse risk among MOGAD patients. Among patients experiencing relapse, prednisolone, first-line immunosuppression, or a combination of those treatments may be associated with decreased risk of future relapse by approximately 2-fold.

Children showed better recovery compared to adults, with adults having a higher risk of relapse. Children with monophasic disease became MOG-Ab negative earlier than relapsing children, but this pattern was not observed in adults.

Predictors of relapse in MOG antibody associated disease: a cohort study

Summary: Male patients with spinal cord involvement at disease onset have a lower risk of relapsing disease and longer latency to first relapse. Steroid treatment for at least 1 month at first attack was also associated with a monophasic disease course. MOG-antibody negative seroconversion was associated with a lower risk of relapse and may help inform treatment decisions and duration.
