Molecular targeted therapy: A new avenue in glioblastoma treatment

Focused Ultrasound-mediated Drug Delivery in Humans - a Path Towards Translation in Neurodegenerative Diseases

Summary: The blood-brain barrier (BBB) plays a crucial role in protecting the brain from harmful molecules but also limits drug delivery, hindering their potential clinical application in neurodegenerative diseases. Recent studies have shown that focused ultrasound with microbubbles (FUS+MB) can temporarily open the BBB, allowing brain-impermeable compounds to enter, offering a promising new platform for drug delivery. While small clinical studies have demonstrated safe BBB opening in patients with Alzheimer's, Parkinson's, and ALS, improved drug delivery has not been achieved in humans. Concurrently, advancements in human induced pluripotent stem cell (hiPSC) modeling technology have allowed for the development of in vitro BBB models derived from Alzheimer's patients, which react to FUS+MB with BBB opening and can be used to study the human BBB response. This review summarizes the limitations of BBB in drug delivery, recent advances in FUS+MB-mediated BBB opening, and potential strategies for translating this technology to improve drug bioavailability in the human brain.
