Examining the paradoxical effects of kratom: a narrative inquiry

Kirsten E. Smith et al.

Summary: This article provides in-depth descriptions of the experiences of regular kratom users through qualitative data. The participants generally experienced both analgesic and stimulatory effects. Most of them reduced their dosages over time, and one planned to quit. Five of the participants met the criteria for kratom-use disorder according to DSM-5. The study also highlights the occurrence of a constellation of symptoms called "the wobbles," which may be a potential manifestation of serotonin syndrome. Participants described tolerance and perceived kratom as generally safe at low-moderate doses, with fewer potential risks compared to pharmaceuticals or illicit drugs.

Changes in the Point-of-Sale Among Vape Shops in Six US Metropolitan Areas Over Time, 2018-2021

Carla J. Berg et al.

Summary: This study examined the regulatory compliance, promotional strategies, and product changes among e-cigarette retail shops in the United States. The findings revealed that as the regulatory policies on e-cigarettes become stricter, there was a decrease in the sale of in-house e-liquid brands and the accommodation of onsite use and sampling, but there was also a decrease in the posting of minimum-age signage. Additionally, there was an increase in the sale of cannabis-related products. These changes highlight the need for comprehensive surveillance to assess the impact of regulations.

Unlocking the potential of TRPV1 based siRNA therapeutics for the treatment of chemotherapy-induced neuropathic pain

Akhilesh et al.

Summary: Chemotherapy-induced neuropathic pain (CINP) is a common clinical complication associated with anti-cancer drugs, affecting nearly 68.1% of cancer patients undergoing chemotherapy. The involvement of TRPV1 in CINP signaling suggests that targeting this nociceptor with siRNA may provide effective therapeutic interventions for managing CINP.

Understanding Kratom Use: A Guide for Healthcare Providers

Marc T. Swogger et al.

Summary: Kratom, a plant native to Southeast Asia, has been used for centuries as a mild stimulant and medicine. However, with its growing popularity in the West, concerns about its safety have been raised by US federal agencies, leading to criminalization in some states and cities. The lack of clinical trials on kratom has resulted in confusion among healthcare providers. Despite this, there is a growing body of peer-reviewed literature that can inform healthcare providers on how to discuss kratom use with consumers, while recognizing that it has not been approved as safe and effective for any disease. One important knowledge gap is the safety of combining kratom with other substances and medications. Therefore, this comprehensive overview aims to provide clarity for healthcare providers and patients.

Kratom alkaloid mitragynine: Inhibition of chemotherapy-induced peripheral neuropathy in mice is dependent on sex and active adrenergic and opioid receptors

Daniel J. Farkas et al.

Summary: The study found that MG has a significant inhibitory effect on oxaliplatin-induced mechanical hypersensitivity in a mouse model, with higher efficacy in male mice. The mechanism of MG involves activity at mu-opioid, alpha 1- and alpha 2-adrenergic receptors, reducing oxaliplatin-induced mechanical hypersensitivity.

Summary: The primary kratom alkaloid mitragynine and its metabolite 7-hydroxymitragynine may produce antinociceptive effects through synergistic actions with adrenergic-alpha(2) receptor (A alpha R-2) and m-opioid receptor (MOR) agonists. Additionally, when combined with A alpha R-2 agonists, mitragynine may also produce hypothermic effects.

Summary: Chemotherapy-induced peripheral neurotoxicity (CIPN) is a common side effect of antineoplastic treatment without effective prevention options, but modulation of the endocannabinoid system may be a promising therapeutic approach. Clinical trials are underway to investigate changes in endocannabinoid levels induced by systemic chemotherapy and the potential role of endocannabinoid system modulation in relieving CIPN symptoms.

Joseph J. Palamar

Summary: The study found that the prevalence of Kratom use in the past year in the U.S. is 0.7%, with a higher likelihood of use among individuals with prescription opioid use disorder. In addition to prescription opioid use disorder, individuals who reported past-year cannabis, cocaine, and prescription stimulant misuse also had a higher likelihood of using Kratom. Further research is needed to understand the reasons for Kratom use and potential risks associated with its use alongside other drugs.

Kratom Use Within the Context of the Evolving Opioid Crisis and the COVID-19 Pandemic in the United States

Walter C. Prozialeck et al.

Summary: Kratom, an evergreen tree native to Southeast Asia, has been used for generations in the region to combat fatigue, improve mood, relieve pain, and manage symptoms of opioid withdrawal. Its recent emergence as an important herbal agent in the United States coincides with the increase in opioid abuse and dependence, as well as restrictions on prescription opioids. This has led to a surge in interest in kratom as a potential harm reduction agent for treating pain and opioid use disorder.
