Summary: Kidney disease is a major global health concern that requires effective therapies, and Toll-like receptors (TLR) 2 and 4 play significant roles in the inflammatory response associated with kidney diseases. TLR2/4 signaling is elevated by SGLT2 and ER stress, leading to inflammation and disease progression. The interactions between TLR2/4, ER stress, and SGLT2 suggest that TLR2/4 may be a potential therapeutic target for kidney disease. Preclinical studies using SGLT2 inhibitors have shown promising results in inhibiting TLR2/4 signaling and reducing inflammation in kidney diseases.

Phloretin inhibits glucose transport and reduces inflammation in human retinal pigment epithelial cells

Summary: Age-related macular degeneration (AMD) is caused by chronic inflammation, possibly fueled by high glucose levels. Phloretin, found in apples, has anti-inflammatory effects and inhibits glucose uptake. In this study, ARPE-19 cells were exposed to lipopolysaccharide (LPS) and then treated with phloretin. Phloretin reduced the release of pro-inflammatory cytokines and increased intracellular reactive oxygen species levels. The Nrf2 pathway was found to be the main mechanism of phloretin's anti-inflammatory effects.

Evaluating the potential of tauroursodeoxycholic acid as add-on therapy in amelioration of streptozotocin-induced diabetic kidney disease

Summary: This study evaluated the potential and underlying molecular mechanism of the bile acid tauroursodeoxycholic acid (TUDCA) as a monotherapy and as an add-on therapy to telmisartan for treating diabetic kidney disease (DKD). The results showed that the addition of TUDCA to telmisartan treatment was more effective in restoring renal function and suppressing apoptotic and fibrotic signaling compared to monotherapies. This suggests that TUDCA, traditionally used in Chinese medicine, could be a potential renoprotective compound when used in combination with telmisartan.

Renin-angiotensin-aldosterone pathway modulators in chronic kidney disease: A comparative review

Summary: Chronic kidney disease is a complex condition with various underlying causes. Current pharmacotherapeutic treatment options aim to slow disease progression and improve quality of life but cannot achieve complete cure. The first-line treatment for controlling blood pressure in chronic kidney disease is the use of renin-angiotensin-aldosterone system modulators. These modulators, including direct renin inhibitors, angiotensin-converting enzyme inhibitors, and angiotensin II receptor blockers, have different structures and mechanisms of action, leading to varying treatment outcomes. The choice of modulator depends on patient presentation, comorbidities, treatment availability and affordability, and healthcare provider expertise. Comparative studies between these modulators are currently lacking, which could provide valuable information for healthcare providers and researchers. This review compares the direct renin inhibitor (aliskiren) with angiotensin-converting enzyme inhibitors and angiotensin II receptor blockers, aiming to identify specific areas of interest for intervention and the best possible treatment option.
