Differing strategies for sudden death prevention in hypertrophic cardiomyopathy

Summary: Sudden death (SD) is the most visible and feared complication of hypertrophic cardiomyopathy (HCM). The introduction of primary prevention ICDs and risk stratification algorithms has significantly reduced HCM-related mortality. However, differences in risk stratification guidelines between countries and societies may lead to different assessments of SD risk in patients.

Evolution and triggers of defibrillator shocks in patients with arrhythmogenic right ventricular cardiomyopathy

Summary: This study analyzed the cumulative burden, evolution, and potential triggers of appropriate ICD shocks in ARVC patients during long-term follow-up. The results showed that the long-term risk of appropriate ICD shocks remained high in ARVC patients, with a preference for daytime occurrence and no seasonal preference. The most common triggers for appropriate ICD shocks were physical activity, inflammation, and hypokalemia.

Summary: Hypertrophic cardiomyopathy (HCM), a common and often inherited primary cardiac disease, can be effectively treated with various options that significantly improve patient outcomes. Innovations in risk stratification, surgical intervention, anticoagulation prophylaxis, and ablation techniques have reduced morbidity and mortality rates. Palliative pharmacological strategies can control symptoms in the short-term, but have limited impact on long-term clinical course.
