Staphylococcus aureus hitchhiking from colonization to bacteremia via Candida within ICU infection prevention studies: a proof of concept modelling

Summary: The study investigates the interaction between Candida and Staphylococcus colonization in the pathogenesis of bacteremia. The results show that the co-colonization of Candida and Staphylococcus improves the model fit, and the model coefficients related to exposure to antibacterial and antifungal agents are contrary to Candida colonization. However, the coefficients related to exposure to antibacterial and antifungal agents are weaker in their association with Staphylococcus colonization.

Establishing the safety of selective digestive decontamination within the ICU population: a bridge too far?

Summary: In this study, the researchers investigated the effectiveness of infection prevention interventions within the ICU setting. They found that selective digestive decontamination (SDD) was highly effective in preventing ICU infections. However, the results differed significantly when comparing randomized controlled trials with cluster randomized trials, suggesting a possible spillover effect of SDD.

Summary: Selective digestive decontamination (SDD) regimens, including topical antibiotic prophylaxis (TAP) and protocolized parenteral antibiotic prophylaxis (PPAP), are highly effective in preventing ICU-acquired infections within randomized concurrent control trials. The concept of SDD suggests potential population benefit, but natural experiments using non-concurrent controls indicate overall population harm associated with higher event rates in the ICU context with SDD use.
