Laparoscopic bile duct plasty for hilar bile duct stenosis (HBDS) in patients with congenital biliary dilatation: Diagnosis of HBDS by preoperative MRCP and laparoscopic strategy to relieve HBDS

Yoichi Nakagawa et al.

Summary: Congenital biliary dilatation (CBD) is a disease associated with pancreaticobiliary maljunction. This study retrospectively analyzed the records of patients who underwent primary surgery for CBD between 2013 and 2021. The results showed that hilar bile duct stenosis (HBDS) occurred in 52% of the cases, and preoperative magnetic resonance cholangiopancreatography (MRCP) had a sensitivity of 96% and a specificity of 74% in detecting HBDS. The intraoperative findings and preoperative imaging can be complementary in planning management to reduce long-term complications.

Robotic versus laparoscopic surgery for choledochal cyst in children with aberrant hepatic ducts: A retrospective study

Xiaolong Xie et al.

Summary: This study compared the safety and effectiveness of robotic and laparoscopic surgery in treating pediatric patients with choledochal cysts associated with aberrant hepatic duct. It found that robotic surgery can achieve the same results as laparoscopic surgery while reducing the surgical difficulty and promoting faster recovery.

Hepatolithiasis caused by right hepatic artery branches forming an arterial ring compressing the common hepatic duct

Yifan Wang et al.

Summary: Anatomic variations of the hepatic artery can lead to biliary obstruction, although the incidence is low. In this study, we report a case of biliary obstruction and hepatolithiasis caused by extrinsic compression of the common hepatic duct by an arterial ring formed by the anterior and posterior branches of the right hepatic artery. We review the existing literature on this topic and discuss diagnostic and treatment strategies.

Summary: Anatomical variations of AHDs play a significant role in hepatic biliary injury during cholecystectomy, with the topographical relationship between right AHDs and left AHDs showing differences in relation to the hepatic arteries.

Laparoscopic repositioning of an aberrant right hepatic artery and hepaticojejunostomy for pediatric choledochal cyst: A case report

Ryuta Masuya et al.

Summary: Aberrant right hepatic artery crossing the common hepatic duct ventrally is a common abnormality that can lead to postoperative anastomotic stenosis. Dorsal repositioning of the artery is effective in preventing and managing this complication.

Risk factors and outcomes of bile leak after laparoscopic surgery for congenital biliary dilatation

Yujiro Tanaka et al.

Summary: This study investigated the risk factors and outcomes of bile leak after laparoscopic surgery for congenital biliary dilatation, findings revealed that hepatic duct diameter at anastomosis is the only risk factor associated with bile leak. While bile leak was more common in patients with a diameter <= 10mm and when surgery was performed by non-qualified surgeons, it did not lead to anastomotic stricture in any of the cases.
