Quantitative Fecal Microbiota Profiles Relate to Therapy Response During Induction With Tumor Necrosis Factor α Antagonist Infliximab in Pediatric Inflammatory Bowel Disease

Summary: This study aimed to investigate the absolute abundances of gut microbiota in relation to the response to induction therapy with infliximab in pediatric inflammatory bowel disease. The results showed that the treatment responsive group had a higher absolute abundance of Bifidobacteriales and a lower absolute abundance of Actinomycetales compared to the nonresponders. However, the level of inflammation according to fecal calprotectin showed no statistically significant association with the absolute abundances of fecal microbiota. The results on relative abundances differed from the absolute abundances.

Summary: The Crohn's disease exclusion diet (CDED) plus partial enteral nutrition (PEN) and exclusive enteral nutrition (EEN) can induce remission in pediatric Crohn's disease. CDED+PEN is better tolerated and able to sustain remission. The remission induced by CDED+PEN is associated with significant changes in inflammatory bowel disease-associated metabolites such as kynurenine, ceramides, amino acids, and others.

The results showed that adding glycerol decreased the pH of the fermentation broth, but the modified medium could alleviate this decrease. Additionally, adding glycerol changed the fermentation patterns and altered the composition of the microbial community.

Therapeutic drug monitoring is preferred over empirically escalating anti-TNF dose or switching therapies. For low-risk luminal CD patients, induction with exclusive enteral nutrition or corticosteroids, and maintenance therapy with immunomodulators are recommended.

Bacterial and Fungal Profiles as Markers of Infliximab Drug Response in Inflammatory Bowel Disease

Intestinal Cetobacterium and acetate modify glucose homeostasis via parasympathetic activation in zebrafish

Summary: Fish have limited capability to utilize carbohydrates compared to mammals. In this study, zebrafish fed with omnivorous and herbivorous diets showed better glucose homeostasis due to enrichment of the commensal bacterium Cetobacterium somerae, which produces acetate. Dietary supplementation of acetate promotes glucose utilization in zebrafish, and the beneficial effect of acetate and C. somerae on glucose homeostasis is mediated through parasympathetic activation. These findings suggest potential strategies for improving fish carbohydrate utilization.
