Blood Pressure Variability After Cerebrovascular Events: A Possible New Therapeutic Target A Narrative Review

Maximiliano A. Hawkes et al.

Summary: Blood pressure variability, resulting from various factors, can worsen secondary brain injury in acute cerebrovascular disease. Studies have shown that increased blood pressure variability is associated with worse outcomes in patients with acute ischemic stroke, intracerebral hemorrhage, and aneurysmal subarachnoid hemorrhage. Therefore, reducing blood pressure variability may be an important therapeutic target.

Effect of an individualized versus standard blood pressure management during mechanical thrombectomy for anterior ischemic stroke: the DETERMINE randomized controlled trial

Benjamin Maier et al.

Summary: The DETERMINE trial aims to assess whether individualized blood pressure management during endovascular therapy for acute ischemic stroke can lead to better functional outcomes compared to standard blood pressure management. The trial will include 432 patients and evaluate both primary and secondary outcomes.

Adverse Outcomes Associated With Higher Mean Blood Pressure and Greater Blood Pressure Variability Immediately After Successful Embolectomy in Those With Acute Ischemic Stroke, and the Influence of Pretreatment Collateral Circulation Status

Dacheng Liu et al.

Summary: This study demonstrates that higher postprocedural blood pressure is associated with unfavorable 90-day outcomes after successful endovascular thrombectomy, particularly in patients with poor collaterals.

Prolonged Duration of Blood Pressure Drops During General Anesthesia Is Associated With Worse Outcomes After Mechanical Thrombectomy

Chao Xu et al.

Summary: This study found that prolonged episodes of intraprocedural MAP lowering during mechanical thrombectomy (MT) were more likely to result in poor outcomes for large vessel occlusion (LVO) patients treated with general anesthesia, which may help guide hemodynamic management during procedures under general anesthesia.

Systolic blood pressure variability following endovascular thrombectomy and clinical outcome in acute ischemic stroke: A meta-analysis

Gaurav Nepal et al.

Summary: Short-term systolic blood pressure variability (BPV) parameters (such as SD, CoV, SV) are associated with clinical outcomes in acute ischemic stroke patients following endovascular thrombectomy (EVT), including achieving good functional outcomes at 3 months and higher Rankin Scale scores. High BPV, however, is not significantly associated with symptomatic intracranial hemorrhage and all-cause mortality at 90 days, and the association with early neurological deterioration remains inconclusive.

Hemodynamic Status During Endovascular Stroke Treatment: Association of Blood Pressure with Functional Outcome

Min Chen et al.

Summary: This study found that intraprocedural pre-recanalization blood pressure reductions during endovascular therapy were not associated with functional outcome, but higher pre-recanalization blood pressure was correlated with worse functional outcomes. Prospective randomized controlled studies are needed to determine the feasibility of blood pressure as a treatment target for modifying outcomes.

Fasting Normoglycemia after Intravenous Thrombolysis Predicts Favorable Long-Term Outcome in Non-Diabetic Patients with Acute Ischemic Stroke

Marcin Wnuk et al.

Summary: The study demonstrated that fasting normoglycemia after intravenous thrombolysis for acute ischemic stroke is associated with a favorable long-term outcome, and it acts as an independent protective factor specifically for non-diabetic patients.
