Summary: Dysfunctional hepatic metabolism is associated with various diseases, such as non-alcoholic fatty liver disease, which can progress to hepatic fibrosis and is closely related to insulin resistance and cardiovascular diseases. The liver secretes metabolites, biomolecules, and microRNAs (miRNAs) that affect metabolic processes in both the liver and peripheral tissues. This review summarizes the involvement of liver-derived miRNAs in biological processes and explores their communication with other tissues in the progression of metabolic diseases. Moreover, it identifies the main molecular targets of miRNAs, providing insights into the underlying mechanisms of metabolic diseases and suggesting the potential use of circulatory miRNAs as prognostic predictors and therapeutic targets.

There are different methods to evaluate pancreatic fat content, but no gold standard for diagnosis and identification of specific types of pancreatic fat. Pancreatic fat storage is associated with chronic inflammation and several diseases, including pancreatitis, diabetes, and pancreatic cancer. Different therapeutic approaches have been proposed, but evidence is lacking.

Summary: Pancreatic cancer is difficult to detect and treat, resulting in poor survival outcomes. The increasing incidence of this cancer is related to aging populations, obesity, and pancreatitis. Risk factors for pancreatic cancer include signaling from fat cells, elevated levels of intrapancreatic adipocytes, inflammation from immune cells, and fibrosis caused by pancreatic obstruction and cell lysis. Further studies are needed to understand the role of adipocytes in the pre-cancerous niche and their implications in mouse models.

The impact on early risk, particularly in individuals with the thin-on-the-outside-fat-on-the-inside phenotype, is not yet determined.

Contribution of Liver and Pancreatic Islet Crosstalk to β-Cell Function/Dysfunction in the Presence of Fatty Liver

Summary: Tissue-to-tissue crosstalk plays a crucial role in regulating organ function, especially for pancreatic beta-cells and the liver. Communication between the two tissues allows for fine-tuning of regulation and adaptive responses. However, this crosstalk is disrupted in the presence of metabolic dysregulation. Factors and cargo released by liver and pancreatic beta-cells, including hepatokines and EVs, play important roles in this interaction. Understanding the crosstalk between the liver and pancreatic beta-cells is essential for understanding and developing better prevention and treatment options for both diseases.

Untargeted metabolomics reveals plasma metabolites predictive of ectopic fat in pancreas and liver as assessed by magnetic resonance imaging: the TOFI_Asia study

Summary: This study identified candidate markers of visceral and ectopic fat through untargeted metabolomics, showing that these markers improved fat level prediction over clinical markers. Several plasma metabolites were associated with liver fat level and VAT/SAT ratio independent of age and adiposity-related parameters, while pancreatic fat deposition was only associated with increased sulfolithocholic acid, independent of age.
