Current Strategies for Exosome Cargo Loading and Targeting Delivery

Haifeng Zeng et al.

Summary: Extracellular vesicles, including ectosomes and exosomes, have emerged as promising natural carriers for drug delivery. Exosomes, which are 30-100 nm in diameter and possess a lipid bilayer, have high biocompatibility, stability, and low immunogenicity, making them desirable for cargo delivery. Loading cargo into exosomes, however, remains challenging despite various strategies developed. This review provides an overview of current cargo delivery strategies using exosomes and summarizes recent approaches for loading small-molecule, nucleic acid, and protein drugs into exosomes, offering insights for more efficient drug delivery.

Exosomes combined with biomaterials in the treatment of spinal cord injury

Xuanxuan Zhang et al.

Summary: Spinal cord injury (SCI) is a serious and disabling disease that often leads to sensory and motor dysfunction, accompanied by various secondary outcomes. The main treatments for SCI include surgery, drugs, and rehabilitation. Cell therapy and exosomes have shown promise in treating SCI, but the therapeutic effect is still controversial. Combining biomaterial scaffolds and exosomes can enhance their delivery to the injury site and improve their viability.

Glutathione-modified macrophage-derived cell membranes encapsulated metformin nanogels for the treatment of spinal cord injury

Qi Yu et al.

Summary: In spinal cord injury (SCI) treatment, the development of a drug preparation that can penetrate the blood-spinal cord barrier (BSCB) and reach the injury site is crucial for therapeutic effectiveness. In this study, a glutathione-modified macrophage-derived cell membrane encapsulating metformin nanogels (Met-CNG-GSH) was prepared, demonstrating slow release, good targeting effect, and therapeutic effectiveness in alleviating oxidative stress, inflammation, and apoptosis.

Stem cell-based therapy for human diseases

Duc M. Hoang et al.

Summary: Recent advancements in stem cell technology have provided new treatment options for patients with untreatable diseases and disorders. Human pluripotent stem cells (hPSCs) and multipotent mesenchymal stem cells (MSCs) have emerged as important players in regenerative medicine. Clinical applications of hPSCs and MSCs derived from different tissues, such as bone marrow, adipose tissue, and umbilical cord, have shown promising results in treating neurological disorders, pulmonary dysfunctions, metabolic/endocrine-related diseases, reproductive disorders, skin burns, and cardiovascular conditions. The mechanisms underlying the therapeutic effects of these stem cells vary depending on their tissue origin.

Summary: In this study, it was demonstrated that a locally administered injection of USC-Exo embedded in hydrogel can deliver ANGPTL3 to the injured spinal cord region. Additionally, the administration of human USC-Exo could enhance spinal cord neurological functional recovery by promoting angiogenesis. Mechanistic studies revealed that ANGPTL3 enriched in USC-Exo is required for their ability to promote angiogenesis through the PI3K/AKT signaling pathway.

Bone marrow mesenchymal stem cell-derived exosomal microRNA-125a promotes M2 macrophage polarization in spinal cord injury by downregulating IRF5

Qing Chang et al.

Summary: B-Exo promotes recovery of locomotor function and M2-phenotype polarization, inhibits neuronal apoptosis and degeneration, as well as the inflammatory response in rats with SCI. IRF5 expression is reduced after B-Exo treatment, and it promotes macrophage polarization towards M1 phenotype and secretion of inflammatory factors. There is a binding relationship between miR-125a and IRF5, with the knockdown of miR-125a in B-Exo leading to increased IRF5 expression and attenuation of the neuroprotective effect against SCI.
