Management of Advanced Pulmonary Sarcoidosis

Rohit Gupta et al.

Summary: Advanced sarcoidosis is a form of sarcoidosis that poses a significant risk of organ function loss or death, with the lung being the most commonly affected organ. It can lead to complications such as pulmonary fibrosis and bronchiectasis. Although it affects a small proportion of patients, it is the leading cause of poor outcomes.

The utility of neutrophil-to-lymphocyte ratio determined at initial diagnosis in predicting disease stage and discriminating between active and stable disease in patients with sarcoidosis: cross-sectional study

Sumeyye Alparslan Bekir et al.

Summary: This study evaluated the utility of neutrophil-lymphocyte ratio (NLR) in predicting the severity of sarcoidosis and distinguishing between active and stable disease. The results showed that NLR was higher in patients with active and advanced disease stages, making it a valuable marker for monitoring disease activity and progression.

Combined Sarcoidosis and Idiopathic Pulmonary Fibrosis (CSIPF): A New Phenotype or a Fortuitous Overlap? Scoping Review and Case Series

Laura Bergantini et al.

Summary: Idiopathic pulmonary fibrosis (IPF) and sarcoidosis are two distinct diseases, but there is a unique phenotype called combined sarcoidosis and idiopathic pulmonary fibrosis (CSIPF). A review of the literature and a study of a CSIPF patient cohort revealed that CSIPF patients have a higher prevalence of males and are mostly ex-smokers. Despite receiving antifibrotic treatment, CSIPF patients still experience functional impairment during a two-year follow-up.

Distinct Airway Involvement in Subtypes of End-Stage Fibrotic Pulmonary Sarcoidosis

Stijn E. Verleden et al.

Summary: In pulmonary fibrotic sarcoidosis, different subtypes show varying levels of involvement in the large airways, but terminal bronchioles are universally lost. This suggests that small airways loss plays an important role in the pathophysiological features of fibrotic pulmonary sarcoidosis.

S100A2 Silencing Relieves Epithelial-Mesenchymal Transition in Pulmonary Fibrosis by Inhibiting the Wnt/β-Catenin Signaling Pathway

Guichuan Huang et al.

Summary: Increased expression of S100A2 was observed in lung tissues of PF patients, and downregulating this protein alleviated PF progression by inhibiting EMT. Targeting S100A2 may be a promising strategy for treating PF and other EMT-associated diseases.

Lung transplantation for sarcoidosis: outcome and prognostic factors

Jerome Le Pavec et al.

Summary: This study retrospectively reviewed data from 112 patients with pulmonary sarcoidosis who underwent lung or heart-lung transplantation, finding that post-transplant survival rates were similar to those with other indications for lung transplantation. Older age and extensive pre-operative lung fibrosis were the main factors associated with worse survival.

Monocytes in sarcoidosis are potent tumour necrosis factor producers and predict disease outcome

Rico Lepzien et al.

Summary: The study found an increase in pulmonary monocyte-derived cells in sarcoidosis patients, which was associated with the development of chronic disease. Highly inflammatory cells were observed in the blood and BAL of sarcoidosis patients. The frequency of BAL monocytes/monocyte-derived cells producing TNF without exogenous stimulation at diagnosis increased in patients who developed progressive disease.

High expression of mTOR signaling in granulomatous lesions is not predictive for the clinical course of sarcoidosis

Alex Pizzini et al.

Summary: This study analyzed 58 patients with sarcoidosis and found activated mTORC1 signaling in all granulomas. The phosphorylation of mTOR was correlated with its downstream effectors S6K1 and 4EBP1, but not with Ki-67 expression. The results support the significant role of mTOR in granuloma formation in sarcoidosis.

Genetics in sarcoidosis

Paolo Spagnolo et al.

Summary: Epidemiological and clinical observations suggest a genetic predisposition to sarcoidosis. Recent genetic studies have identified associations with disease activity and prognosis, highlighted the importance of integrating genetic information when assessing the relationship between sarcoidosis and environmental exposures, and revealed that the heritability of sarcoidois is only 49%, indicating the presence of other important contributors to disease risk.

Summary: Lung transplant is a crucial treatment option for patients with advanced lung diseases, but there are challenges and ethical dilemmas in the process of referral, evaluation, and candidacy determination. The International Society for Heart and Lung Transplantation has updated consensus documents to assist providers in identifying, evaluating, and selecting potential candidates for lung transplant, addressing ethical considerations, and acknowledging variability in risk acceptance among transplant centers.
