Adverse perinatal outcomes associated with timing of initiation of antiretroviral therapy: Systematic review and meta-analysis

Summary: There is an association between the timing of ART initiation during pregnancy and perinatal outcomes. Starting ART before pregnancy is associated with an increased risk of preterm birth. In low- and middle-income countries, the timing of ART initiation has no significant association with adverse perinatal outcomes.

Summary: This systematic review and meta-analysis found that pregnant women living with HIV on antiretroviral therapy have an increased risk of preterm birth, low birthweight, and small-for-gestational age in high resource countries compared with HIV-negative controls. The risk of preterm birth and low birthweight is also higher in low income countries for these women.

Use of antiretroviral therapy in pregnancy and association with birth outcome among women living with HIV in Denmark: A nationwide, population-based cohort study

Summary: Pregnant women living with HIV in Denmark receive generally good antiretroviral therapy, with combination treatment of nucleoside reverse transcriptase inhibitor and protease inhibitor being the most common regimen during pregnancy. The study found that initiation of antiretroviral therapy in the first trimester may be associated with poor fetal growth, and the association between antiretroviral therapy use in pregnancy and adverse birth outcomes may be partly explained by maternal risk factors.

Summary: In women with HIV undergoing combination antiretroviral therapy (cART) during pregnancy, initiating treatment earlier is associated with increased risk of preterm birth and decreased risk of small-for-gestational age birth compared to late treatment initiation. However, there is no significant difference in fetal death between early and late treatment initiators.

Summary: This study assessed the risk of adverse perinatal outcomes in pregnant women living with HIV receiving protease inhibitor-based antiretroviral therapy (PI-ART) and compared the risks associated with different PI-ART regimens. The meta-analyses showed that PI-ART was associated with an increased risk of small for gestational age (SGA) and very SGA (VSGA), but not preterm birth (PTB) or other perinatal outcomes. There were no significant differences in perinatal outcomes between different PI-ART regimens.
