Health economic evaluation of endodontic therapies

Summary: This review introduces the main concepts of health economic analysis, systematically reviews the existing economic literature on endodontic treatments, and suggests further action for the community. The identified evidence on the health economics of endodontic therapies is limited and has some limitations. However, maintaining pulpal vitality is preferred over root canal treatment for cost-effectiveness, retaining teeth is usually more cost-effective than extraction and replacement, and endodontic retreatment may be clinically indicated but not always cost-effective and should be carefully considered.

Outcome of selective root canal retreatment-A retrospective study

Summary: Selective root canal retreatment shows favorable clinical and radiographic outcomes in the majority of cases. Unretreated roots rarely develop new periapical lesions based on radiographic evidence. None of the treatment-related parameters have a direct influence on the outcome of the retreated roots. The survival rate at 12-48 months after retreatment is 91.5%.

Tooth survival after endodontic treatment

The study found that tooth restoration is the most important factor associated with the success of root canal treatment. Indirect restorations were found to have a higher survival rate compared to direct restorations.

Effectiveness of 1-Visit and Retreatment of Teeth with Persistent/Secondary Endodontic Infection: A Randomized Clinical Trial with 18 Months of Follow-up

Summary: The study compared the effectiveness of endodontic retreatment in 1 visit versus 2 visits on the reduction of bacteria and other parameters in teeth with posttreatment apical periodontitis over an 18-month period. Results showed differences in bacterial load and LTA levels between the two groups, but no significant differences in endotoxin levels and periapical lesion volume. Both treatment methods exhibited equally favorable periapical healing at 18 months.

The global prevalence of apical periodontitis: a systematic review and meta-analysis

Summary: Half of the adult population worldwide have at least one tooth with apical periodontitis. The prevalence of AP is greater in samples from the dental care services, but it is also high amongst community representative samples from the general population. The present findings should bring the attention of health policymakers, medical and dental communities to the hidden burden of endodontic disease in the population worldwide.

Four-year Outcome of Nonsurgical Root Canal Retreatment Using Cone-beam Computed Tomography: A Prospective Cohort Study

Summary: This prospective study investigated the 4-year outcome and prognostic factors of nonsurgical root canal retreatment by measuring the volumetric change of periapical radiolucencies on CBCT scans. The study found a significant reduction in periapical radiolucencies at 4 years postoperatively, with tooth type identified as a predictor of outcome. Overall, the retreatment of endodontically treated teeth showed predictable and successful results.
