Challenging Contrast-Enhanced Mammography-Guided Biopsies: Practical Approach Using Real-Time Multimodality Imaging and a Proposed Procedural Algorithm

Summary: Contrast-enhanced mammography (CEM) is a promising breast imaging technique that uses dual-energy digital mammographic images with the help of intravenous iodine-based contrast material. CEM-guided biopsy, introduced in 2019 and approved by the U.S. FDA in 2020, allows direct sampling of suspicious enhancements visible on or predominantly on recombined CEM images. This article presents our initial experience and suggests a procedural algorithm to address technical difficulties and improve targeting success.

Contrast-Enhanced Mammography-Guided Breast Biopsy: Single-Center Experience

Summary: This single-center study included 50 women with 51 contrast-enhanced mammography (CEM)-enhancing lesions without sonographic or mammographic correlate. CEM-guided biopsy was performed, with a technical success rate of 92.2% and a cancellation rate of 9.8%. The mean biopsy time was 16.6 minutes. All biopsies showed concordant pathology, with 25 benign, 10 high-risk, and 11 malignant lesions. No malignancy was found in nonvisualized or benign lesions during follow-up.

Contrast-enhanced mammography-guided biopsy: technical feasibility and first outcomes

Comparison of False-Positive Versus True-Positive Findings on Contrast-Enhanced Digital Mammography

Summary: This study compared imaging characteristics of false-positive and true-positive findings on contrast-enhanced digital mammography (CEDM). The results showed that lesions present on both low-energy and iodine images were more likely to be true-positive, especially when the mammographic finding was an asymmetry or calcification. Additionally, calcifications with associated enhancement had a high malignancy rate.
